Acknowledgments
A large debt is owed to Judith Hickey, RN who has faithfully and thoroughly collected records and made arrangements for patients 
to be seen and operated upon for many years and our LSUHSC of Neurosurgery Administrator, Latricia Jackson who has recently 
moved to another department post-Katrina. Willie Coleman our Laboratory technician passed away shortly after Hurricane Katrina 
and we miss him. The work for this edition could not have been completed without the constant diligence of Dr. Kline’s secretary, 
Vanissia Prout. Dr. Leo Happel our neurophysiologist has also provided advice and sometimes directions for the operative recordings 
used in many of the patients reported upon. Especial thanks are also due our medical artist for the First Edition, Eugene New and 
to Barbara Seide for some drawings used in the Second Edition.
Of course, especial thanks to our patients and their referring physicians our residents and fellows and other faculty, especially Austin 
Sumner, and of course our own families without whom this Second Edition would not be possible.
