P ad rõ es R ad io ló g ic o s B ás ic o s As cinco densidades em ordem de aumento de densidade: Ar, gordura, Líquido, Osso, Metal. Terminologia para descrição de lesões ósseas • Diminuição generalizada da densidade óssea (osteopenia); • Diminuição focal - Lesão lítica ou osteolítica (hipodensa, radiolucente); • Aumento generalizado da densidade óssea (esclerose óssea); • Lesão esclerótica (hiperdensa, radioopaca); • Alterações na forma ou modelagem dos ossos. Sinais de Lesão Articular • Estreitamento do espaço articular: destruição da cartilagem articular, ocorre em artrites. • Edema das partes moles periarticulares: corresponde à uma sinovite, ou seja, derrame intra-articular ou espessamento da sinóvia. Trauma, inflamação ou infecção. • Osteoporose justa-articular: ocorre em várias condições dolorosas. Desuso pode ser em uma das causas. Importante em AR e TB óssea. American Journal of Roentgenology, v. 97, n. 3, p. 708-718, 1966. 27/02/2016 2 Neoformação Óssea • A Neoformação óssea pode originar-se de elementos ósseos normais ou de células tumorais. • Neoformação reativa é ordenada e organizada. É um espessamento e aumento de densidade de uma trabécula pré-existente. • Neoformações tumorais dentro dos ossos podem aparecer como “ninhos de marfim” (tecido osteogênico; osteossarcoma ou osteoblastoma) ou múltiplos flóculos (ou manchas – tecido condrogênico; condrossarcoma ou condroblastoma). Tumor new bone due to osteosarcoma. New bone formation is seen within the anterior portion of the third rib due to osteosarcoma. The dense homogeneous density is characteristic of new bone formation of osteogenic tumor. Tumor new bone formation due to chondrosarcoma. The large mass extending from the ramus of the pubis contains multiple flecks of calcification. This type of calcification is characteristic of cartilaginous calcification and indicates the presence of a chondrogenic tumor. Reação Periosteal • Ocorrências comuns em lesões ósseas. • Reposta à estímulos lesivos. • O periósteo normal não é visível em radiografias, mas quando lesado, há percepção de alterações. Imagens de RM da coxa de aspecto normal. A: Imagem axial adquirida na ponderação T1 evidencia fina lâmina concêntrica de sinal intermediário na topografia do periósteo (seta). B: Imagem coronal na ponderação T1, em que a seta também indica a topografia do periósteo normal. Reações Periosteais Sólidas • Camada única de osso neoformado maior que 1 mm de espessura, forma-se junto à superfície cortical. • Uniformes em densidade, a camada neoformada é homogênea. • Podem haver diferenças entre pacientes. • É indicativo de processo benigno. 27/02/2016 3 Reações Periosteais Sólidas • Apesar de poder crescer em tamanho, a uniformidade altera-se minimamente. • Pode ser fina ou espessa, com qualquer grau de densidade, podendo até mesmo ter uma superfície plana ou ondulada. • Espessamento e densidade relacionam-se à agressividade da irritação. Reações Periosteais Sólidas • Duração pode afetar interpretação: irritações leves agindo por longos períodos podem parecer irritações agressivas agindo por curto tempo. • Independente do tempo e intensidade, a resposta periosteal em condições benignas usualmente será plana e uniforme. Reações Periosteais Sólidas - Exemplos • Granuloma eosinofílico • Fraturas • Osteomielite • Hemorragia • Osteoartropatia pulmonar hipertrófica • Osteoma osteoide • Doenças Vasculares • Doenças de depósito Solid periosteal reaction (osteoid osteoma). Reação Ondulada Espessa • Geralmente ocorre com varicosidades de longa data ou doenças arteriais. • Geralmente possui 1 cm ou mais de espessura. • A superfície é áspera e ondulante. • O mecanismo de formação é desconhecido. 27/02/2016 4 Dense undulating periosteal reaction secondary to longstanding peripheral vascular disease. There is periosteal new bone formation most noticeable on the lateral aspect of the fibula. The external surface is undulated. The periosteal reaction is quite thick and almost homogeneous and Indicate as benign process. Reações Ondulada Fina • Localizam-se primariamente no aspecto côncavo de ossos longos. • Osteoartropatia pulmonar é o melhor exemplo. • Não são densamente ossificadas e podem ser associadas com processos benignos ou malignos da cavidade torácica. • Mecanismo de crescimento é desconhecido. Dense undulating periosteal reaction due to pulmonary osteoarthropathy. The periosteal reaction is quite thick. Initially, a small dense periosteal reaction may be noted but as the condition progresses it becomes much thicker and irregularly calcified. (A) Standard roentgenogram. (B) Magnified roentgenogram. Reação Elíptica Espessa • Varia de 2 mm até 1 cm de espessura. • São mais espessas ao centro e afunilam-se nas laterais. • Estão presentes em osteoma osteoide cortical de longa data. • Raramente, tumores de células redondas (Sarcoma de Ewing ou Sarcoma de células do retículo) tratadas por irradiação podem ter esse tipo de reação. No último, porém, a reação é permeada por áreas osteolíticas, que são características do crescimento de processo maligno. Dense elliptic periosteal reaction due to long standing osteoid osteoma. The solid nature of the reaction indicates benignancy. Reação Fusiforme • Acontece em condições benignas antigas, como doenças de depósito e osteomielite crônica. • Usualmente possui alguns mm de espessura e é irregularmente densa. • Não ocorre com malignidade. 27/02/2016 5 Periosteal cloaking due to Gaucher's disease. Triângulo de Codman • Inicialmente descrito por Ribbert em 1914, que acreditava que a elevação era causada por massa expansiva. • Era associado à processos malignos: Sarcoma de Ewing e Osteossarcoma. • Atualmente considera-se que pode indicar processos benignos e malignos. • Microscopicamente, células tumorais não estão na área da reação, de fato, quando invadido por tumores, o triângulo desaparece. • Contrastantemente, pus e sangue podem elevar o periósteo e causar o triângulo. Codman's triangle secondary to osteosarcoma. A Codman's triangle is noted at the proximal end of an osteolytic lesion in the distal end of the femur. (Courtesy of Katz L, New Haven, CT) Reações Periosteais Descontínuas • São causadaspor condições ativas (tumores malignos, infecções, hemorragias repetidas). • A descontinuidade é resultado de processo rapidamente progressivo. • Sua principal característica é a perda da estabilidade, alteram-se constantemente. 27/02/2016 6 Reação Lamelada • Ocorre em condições ativas como osteomielite aguda e tumores malignos. • A lamelação é resultado de crescimento não contínuo. • De acordo com alguns autores, uma lamelação pode-se formar em menos de uma semana. • Outros autores consideram que as lamelações são resultados da altenância de fases de crescimento rápido e lento. • Durante o crescimento lento, o periósteo possui tempo para formar uma camada óssea análoga a um invólucro de infecção. • Durante o crescimento rápido, não há formação de osso. • Sangramento para dentro do periósteo também influencia nesse padrão. Reação Perpendicular (Cabelos em Pé) • Espículas com orientação perpendicular à superfície óssea. • Geralmente tumores malignos elevam o periósteo. • De acordo com alguns autores, a reação ocorre ao longo das bandas fibrosas (Sharpey) entre o periósteo e a cortical. • Também pode haver crescimento ósseo em canais vasculares periosteais. • Processos agressivos. Lateral radiograph of lower leg shows bony spicules emanating perpendicular to cortex (arrows). 27/02/2016 7 Reação Perpendicular (Raios de Sol) • Reação periosteal divergente. • Espículas apontam para o epicentro da lesão óssea. • Relaciona-se à processos malignos (osteomielite aguda, tumores malignos). • Pode aparecer em lesões benignas (osteoblastoma e hemangioma). Perpendicular (sunburst) periosteal reaction due to osteosarcoma. There is marked destruction in the distal end of the femur with considerable soft tissue extension. Reações Amorfas • Situadas principalmente entre o novo osso periosteal e a cortical do osso principal, esses depósitos podem estar presentes na extensão do tumor mais frequentemente que na resposta periosteal ao tumor. • Geralmente ovais ou esféricos, variam de alguns mm até cm em cortes axiais. • Geralmente denotam malignidade. Amorphous deposits of tumor new bone and extensive destruction of the neck of the femur due to osteosarcoma. The small deposit of amorphous calcification is often present with malignant bone tumors. It probably represents a deposit of tumor new bone rather than periosteal reaction. 27/02/2016 8 Patterns of periosteal reaction. Acidentes Ósseos • Capítulo: cabeça articular pequena e redonda (capítulo do úmero); • Côndilo: área articular arredondada, que geralmente ocorre em pares (côndilos lateral e medial do fêmur); • Crista: crista do osso (crista ilíaca); • Epicôndilo: proeminência superior a um côndilo (epicôndilo lateral do úmero); Acidentes Ósseos • Fóvea: área plana lisa, geralmente coberta por cartilagem onde um osso articula-se com outro (fóvea costal superior de uma vértebra – articulação com costela); • Forame: passagem através de um osso (forame obturado); • Fossa: área oca ou deprimida (fossa infaespinal da escápula); • Sulco: depressão ou escavação alongada (sulco do nervo radial do úmero); Acidentes Ósseos • Cabeça: extremidade articular grande e redonda (cabeça do úmero); • Linha: elevação linear (linha para o músculo sóleo na tíbia); • Maléolo: processo arredondado (maléolo lateral da fíbula); • Incisura: entalhe na borda de um osso (incisura isquiática maior); Acidentes Ósseos • Protuberância: projeção de um osso (protuberância occipital externa); • Espinha: processo semelhante a um espinho (espinha da escápula); • Processo espinhoso: parte que se projeta semelhante a um espinho, apófise (processo espinhoso de uma vértebra); • Trocanter: elevação arredondada grande (trocanter maior do fêmur); Acidentes Ósseos • Tróclea: processo articular que assemelha-se a uma roda ou processo que atua como roldana (tróclea do úmero); • Tubérculo: proeminência pequena e elevada (tubérculo maior do úmero);
