Palavras-chave: Penicilamina. Summary: Elastosis perforans serpiginous (EPS) is a rare skin disease characterized by transepidermal elimination of abnormal elastic fibers. The pathogenesis of EPS is still unclear. Three forms of EPS are reported: idiopathic, associated with certain genetically determined disorders of connective tissue and D-penicillamine-induced. The authors report a case of a 20-year-old man with D-penicillamine-induced E P S , and discuss the clinical, histopathologic and electron microscopy aspects. Key words: Penicillamine. A D-penicilamina é um agente quelante do cobre usado no tratamento da doença de Wilson (degeneração hepatolenticular), que impede o acúmulo excessivo do cobre no fígado, cérebro e nos olhos desses doentes. Esse tratamento pode induzir o aparecimento de várias doenças cutâneas, tais como EPS, cútis laxa, pseudoxantoma elástico, anetodermia, pênfigos, penfigóide bolhoso e reações de hipersensibilidade aguda. Relata-se a seguir o caso de um portador de doença de Wilson, em uso de D-penicilamina, há dois anos evoluindo com quadro característico de EPS. RELATO DO CASO Doente de 20 anos, do sexo masculino, branco, com doença de Wilson diagnosticada aos nove anos de idade, em uso de D-penicilamina há 11 anos. Ao exame dermatológico apresentava pápulas e placas violáceas de superfície hiperqueratósica e configuração serpiginosa e arciforme, localizadas no pescoço, na axila direita (Figura 1) e simetricamente nas regiões antecubitais (Figura 2). A doença hepática manifestou-se pela primeira vez com quadro de icterícia colúrica e hipocolia fecal que evoluiu para encefalopatia hepática. Na época, os exames evidenciaram ceruloplasmina < 6mg/dl (normal 26- 63mg/dl) e cobre urinário elevado, confirmando a diagnose da doença de Wilson. Foi então introduzida a D- penicilamina na dose de 1250mg/dia, com controle da doença hepática. De antecedentes familiares, negava conhecimento de quadro dermatológico semelhante. O exame histopatológico da biópsia de pele, na coloração hematoxilina-eosina evidenciou, na derme, a presença de vasos dilatados circundados por infiltrado inflamatório moderado linfo-his- tiocitário em toda sua extensão, e, entre as fibras colágenas, notava-se material amorfo granular eosinofílico de eliminação transepidérmica (Figura 3). Na coloração de Weirghert Unna Tanger (WUT), para fibras elásticas, demonstrou-se que esse material se tratava de fibras elásticas degeneradas (Figura 4). O exame de microscopia eletrônica demonstrou fibras D-penicillamine is a copper chelating agent used in the treatment of Wilson disease (hepatolenticular degeneration), which prevents the excessive accumulation of copper in the liver, brain, and eyes of the patient. Such treatment can trigger several cutaneous diseases, such as EPS, cutis laxa, elastic pseudo-xanthoma, anetodermia, pemphigus, bullous pemphigoid, and acute ypersensibility. The case reported is of a Wilson disease patient who has been receiving D-penicillamine for eleven years, and over the last two years has presented clinical features characteristic of EPS. CASE REPORT A white, 20-year old male with Wilson disease diagnosed when he was 9 and who has been receiving D-penicillamine ever since. The dermatological exam showed papules and purplish plaques with hyperkeratosis, serpiginous and arciform, on the neck, right armpit (Figure 1), and symmetrically on anticubital regions (Figure 2). The hepatic disease first manifested itself with jaundice and fecal hipocolia, which developed to hepatic encephalopathy. At that time, the exams showed ceruloplasmina < 6mg/dl (normal 26-63mg/dl) and elevated urinary copper, confirming the Wilson disease diagnosis. D-penicillamine was then commenced at a dose of 1250mg/day, which controlled the hepatic disease. There was no known history of similar dermatological problems in the family. The histophatologic exam of the skin biopsy, colored with hematoxiline-eosin showed the presence of several dilated vasa surround by moderate linpho- hystrocite inflammatory infiltrate throughout their extension in the dermis. It also showed granular amorphous eosinophil material of transepidermal elimination among the collagen fibers (Figure 3). Trough (WUT) coloration, it was demonstrated that the amorphous material was in fact degenerating elastic fibers (Figure 4). The electron microscopy exam demonstrated elastic fibers. Such fibers, however, presented a spiky irregular outline, some of which with reduced lumen and some with increased lumen. In the central position there were fibrilar elements, and its peripheral portion consisted of amorphous elastin matrix (‘lumpy bumpy’). Some of the fibers were surrounded by dense and fibrilar electron material. Cellular inflammation elements, macrophages and lymphocytes,were found together with the altered fibers (Figure 5). The exam also showed alteration in the collagen fibers. Such collagen fibers formed groups composed by twisted fibrils of variable thickness. The habitual period was apparently kept the same (Figure 6). Prior to the exam, electrocoagulation was performed with electric bistoury and laser ray without any improvement or formation of hiperthrophic scar. The proposed treatment was cryotherapy with liquid nitrogen. Two sessions were performed when new lesions appeared around the treated area. The D-penicillamine was then changed to another copper chelating, zinc acetate 175mg/day, without involution of the lesion after a year of treatment. Treatment with topical elásticas alteradas. Essas fibras, entretanto, apresentavam contornos irregulares e espiculados, algumas delas exibindo calibre diminuído, outras, aumentado, com elementos fibrilares ainda presentes em sua porção central e periferia composta por matriz de elastina amorfa (lumpy bumpy). Algumas dessas fibras estavam envolvidas por material elétron denso e fibrilar. Junto às fibras alteradas estavam presentes elementos celulares inflamatórios macrófagos e linfócitos (Figura 5). O exame evidenciou também alteração de fibras colágenas. Essas por vezes formavam feixes compostos por fibrilas de espessura variável e torcidas. A periodicidade habitual era aparentemente mantida (Figura 6). Anteriormente refere ter sido feito uma eletrocoa- gulação com bisturi elétrico e laser sem melhora e formação de cicatriz hipertrófica. O tratamento proposto foi a crioterapia com nitrogênio líquido, tendo sido realizadas duas sessões quando apare- ceram lesões novas ao redor do local tratado. Foi substituída a D-penicilamina por outro quelante de cobre, o acetato de zinco 175mg/dia, sem in- volução das lesões após um ano de seguimento. DISCUSSÃO EPS é doença rara da pele, caracterizada pela eliminação transepidérmica de tecido elástico anormal, sendo descrita pela primeira vez por Lutz, em 1953, 1 e, por Miescher, em 1955.2 Pode ser classificada em três categorias: 1. idiopática; 2. associada a doenças do tecido conectivo, acompanhando síndrome de Down, síndrome de Ehlers Danlos tipo-IV, síndrome de Marfan, síndrome de Rothmund-Thomson, osteogênese imperfeita, pseudoxan- toma elástico, cútis laxa, 3,4,5 morféia 6 e acrogéria;7 3. induzida pela D-Penicilamina durante os tratamentos prolongados para doença de Wilson,8,9,10 artrite reumatóide11,12 ou cistinúria.13 As lesões da EPS são pápulas hiperqueratósicas eritêmato-violáceas, arranjadas de forma circular, arciforme ou serpiginosa, que aumentam com involução central. Em geral são simétricas e localizadas em pescoço, face, cotovelos e joelhos. 14 Podem existir formas disseminadas, costumeiramente associadas à síndrome de Down ou ao uso da D-penicilamina. 15 Embora o caso em questão tenha sido desencadeado pela droga, as lesões apresentavam-se nas localizações clássicas. Ao exame histopatológico observam-se canais de eliminação transepidérmica de material amorfo eosinofílico, que, ao ser corado para fibras elásticas, confirma tratar-se de fibras elásticas degeneradas. Na derme nota-se infiltrado inflamatório mixto com linfócitos, histiócitos, raros neutrófilos e plasmócitos, muitas vezes apresentando células gigantes multinucleadas próximas à retinoic acid 0.005% has been introduced and the course remains to be seen. DISCUSSION EPS is a rare skin disease, characterized by transepidermal elimination of abnormal elastic tissue, first described by Lutz in19531 and Miescher in1955. 2 It can be classified into three categories: 1. Idiopathic; 2. Associated with diseases of the connective tissue, followed by Down Syndrome, Ehlers-Danlos syndrome type-IV, Marfan syndrome, Rothmund-Thomson syndrome, imperfect osteogenesis, elastic pseudo xanthoma, cutis laxa,3,4,5 morphea6 and acrogeria;7 3. Induced by D-penicillamine during extended treatment of Wilson disease,8,9,10 rheumatoid arthritis11,12 or cystinuria. 13 EPS lesions are erythematosus purplish hyperkeratosical papules, disposed in a circle, arcform or serpiginous. They grow with central involution, are normally symmetrical and located on the neck, face, elbows, and knees.14 There may be disseminated forms, generally associated with Down syndrome and with the use of D-penicillamine. 15 The case in question was triggered by drugs, however, the lesions were found in classic locations. The histopathological exam showed channels of transepidermal elimination of eosinophil amorphous material, which on staining was confirmed to be degenerated elastic fibers. A mixed inflammatory infiltrate was observed in the dermis, which consists of lymphocytes, histiocytes, rare neutrophiles and plasmocytes. The electron microscopy exam of EPS induced by D-penicillamine presents, in transverse sections, elastic fibers apparently composed by two regions: a central one, similar to the normal elastic fibers with microfibrils in an amorphous matrix; and an external one that is exclusively composed by amorphous matrix (‘lumpy bumpy’), which exhibits irregular, spiky, outlines. 10,12,17,18,19,20 The same alterations are also found on the apparently normal skin, in cutaneous hemorrhagic lesions, and in other organs (arterial and pulmonary elastic tissue) of patients who are being administered D-penicillamine. 10,12,18,19,20 The ‘lumpy bumpy’ standard of the elastic fibers seems to be specific to the alteration of the elastic tissue induced by D-penicillamine.12 The collagen fibers also present themselves as abnormal, with a significant diameter variation, some of which are quite thick and with a variety of forms.17 The electron microscopy alterations of the case in study were similar to those found in the literature. It was not evidenced the presence of epithelioid cells phagocyting elastic fibers.10,12,17 The latter findings, however, were not relevant to the diagnosis of the disease. D-penicillamine is an amino-acid that contains sulfur (D-b,b-dimetilcistein), which is a penicillindegradation product obtained by hydrolysis. 12,18,21 It was first isolated in the urine of patients with hepato-pathology who had been receiving penicillin. It is an efficient copper, mercury, zinc, and lead chelating. It promotes the urinary excretion of such metals. D-penicillamine is well absorbed via gastro-intestinal (40-70%) and reaches maximum concentration in the plasma between one and three hours after its administration. The hepatic biotransformation is responsible for the greater part of its degradation, and it is excreted via urinary or fecal means. It is mainly used for intoxication with copper, mercury or lead, Wilson disease, cystinuria, rheumatoid arthritis, as well as in some sclerodermis cases and primary biliary cirrhosis.21 The use of D-penicillamine may trigger the onset of several mucous-cutaneous alterations, such as milium, cysts of epidermal inclusion, vulvo ulcerative, scaling, and allergic reactions. It can also trigger the onset of several cutaneous diseases, such as pemphigus, bullous pemphigoid, cicatricial pemphigoid, epidermolysis bullosa, EPS, cutis laxa, elastic pseudo-xanthoma, anetodermia, lupus erythematous, and dermatomyositis.8,10,21,22 The mechanism in which D-penicillamine induces tissue damage in EPS is not well known. The three main theories are: 1. the decrease of copper inhibits the lysil-oxidase enzyme, which is copper-dependent, then, the crossed link of the elastin does not take place which leads to the formation of degenerative elastic fibers; 9,12,17 2. direct inhibition of lysil-oxidase enzyme; 3. direct blockage of the aldeid predecessor of the elastin and collagen crossed link. Therefore, there is an alteration in the maturation of the connective tissue.9,12,18,23,24 The copper dependant lysil-oxidase enzyme entrada dos canais de eliminação transepidérmica. 10,16,17 Esses achados permitem o diagnóstico, mas não diferenciam as três categorias de EPS.16 O exame de microscopia eletrônica na EPS induzida pela D-penicilamina apresenta, nos cortes transversais, fibras elásticas aparentemente compostas por duas regiões: uma central, semelhante à da fibra elástica normal, com microfibrilas embebidas em uma matriz amorfa; e outra externa, composta exclusivamente de matriz amorfa ( lumpy bumpy ), exibindo contornos irregulares e espiculados.10,12,17,18,19,20 Essa alteração é também encontrada na pele aparentemente normal, em lesões hemorrágicas cutâneas e em outros órgãos (tecido elástico pulmonar e arterial) de doentes em uso de D-penicilamina.10,12,18,19,20 O padrão lumpy bumpy das fibras elásticas parece ser específico da alteração do tecido elástico induzido pela D- penicilamina.12 As fibras colágenas também se mostram anormais, com significante variação no diâmetro, algumas bem espessadas e com forma variada.17 As alterações à microscopia eletrônica do caso estudado foram semelhantes às da literatura. Não foi evidenciada a presença de células epitelióides fagocitando as fibras elásticas,10,12,17 esse dado não sendo, entretanto, importante para firmar a diagnose da doença. A D-penicilamina é um aminoácido que contém enxofre (D-b,b-dimetilcisteína), sendo um produto da degradação da penicilina preparado por hidrólise.12,18,21 Foi isolada pela primeira vez em 1953 na urina de pacientes com hepatopatia que estavam recebendo penicilina. É um quelante eficaz do cobre, mercúrio, zinco e chumbo, promovendo a excreção urinária desses metais. A D- penicilamina é bem absorvida por via gastrointestinal (40- 70%) e atinge concentrações máximas no plasma entre uma e três horas após sua administração. A biotransformação hepática é responsável pela maior parte de sua degradação, sendo excretada por via urinária e fecal. É usada principalmente nas intoxicações pelo cobre, mercúrio e chumbo, na doença de Wilson, na cistinúria, na artrite reumatóide e em alguns casos de esclerodermia e cirrose biliar primária.21 O uso da D-penicilamina pode levar ao aparecimento de várias alterações cutâneo-mucosas, tais como formação de mília, cistos de inclusão epidérmica, úlceras vulvares, descamação e reações alérgicas, bem como induzir o aparecimento de várias doenças cutâneas, como pênfigos, penfigóide bolhoso, penfigóide cicatricial, epidermólise bolhosa, EPS, cútis laxa, pseudoxantoma elástico, anetodermia, lúpus eritematoso e dermatomiosite.8,10,21,22 O mecanismo pelo qual a D-penicilamina induz danos ao tecido elástico na EPS ainda não é bem conhecido. Tanakura, of the Pathology Department, for processing the electron microscopy. direta da enzima lisil oxidase; e 3. bloqueio direto dos precursores aldeídos da ligação cruzada da elastina e do colágeno, alterando a maturação do tecido conectivo.9,12,18,23,24A enzima lisil oxidase, cobre dependente, é essencial para a ligação cruzada da elastina e pode ser inibida direta ou indiretamente pela D-penicilamina; porém, a deficiência de cobre por si só, como ocorre na síndrome de Menkes, não está associada à EPS ou à cútis laxa.9,12,23,25A associação do quadro clínico, o uso da droga e as alterações laboratoriais, principalmente a microscopia eletrônica, fazem a diagnose. Os principais diagnósticos diferenciais clínicos e histopatológicos são granuloma anular, poroqueratose de Mibelli, sarcoidose, doença de Kyrle e outras dermatoses perfurantes.8,15,26 A maioria dos pacientes descritos com EPS associada ao uso de D-penicilamina tinha usado a droga por longos períodos, um há 19 anos (tempo médio 11,3 anos), como no paciente em questão (11 anos).12,15 O curso clínico da EPS é imprevisível. Pode ocorrer eventual involução espontânea, resultando em cicatriz atrófica e hipopigmentação local, principalmente nos casos de EPS do tipo idiopática. Nos casos de EPS induzida por D-penicilamina, as lesões costumam ser mais persistentes, raramente ocorrendo involução espontânea.8,9,15,27 Há, contudo, um relato de regressão das lesões com a diminuição da dose de D-penicilamina.8 No doente aqui focalizado não houve regressão das lesões mesmo com a suspensão da droga. O uso de corticosteróides tópicos ou mesmo por infiltração local tem mostrado resultados insatisfatórios. Dermoabrasão, exérese cirúrgica e eletrocirurgia freqüentemente evoluem com cicatrizes queloidais.4,8,15 A crioterapia com nitrogênio líquido é considerada uma boa opção terapêutica,8,15,28 embora muitas vezes pouco tolerada em doentes com lesões extensas e múltiplas.9 Os autores fizeram duas sessões de crioterapia, sem os resultados satisfatórios descritos na literatura. É mencionado também o uso da isotretinoína via oral (0,5mg/kg/dia) durante tempo mínimo de seis semanas, com bons resultados. Alguns doentes podem apresentar exacerbação do quadro clínico e pseudoxantoma elástico após longo período de tratamento com essa droga.9,15 Concluindo, o motivo da apresentação deste trabalho é chamar atenção para a necessidade da avaliação dermatológica periódica dos doentes em uso prolongado da D-penicilamina, pois podem apresentar, além da EPS, outras dermatoses. A história clínica e a microscopia eletrônica diferenciam a EPS induzida pela D-penicilamina das outras formas de EPS. q is essential to the elastin crossed link, and may be directly or indirectly inhibited by D-penicillamine. However, the copper deficiency alone, as in Menkes syndrome, is not associated with EPS or cutis laxa.9,12,23,25 The associationof the clinical features combined with the use of the drug and the laboratorial alterations, mainly seen under electron microscopy, contribute to the diagnosis. The main clinical and histopathological differential diagnosis are granuloma annulare, Mibelli porokeratosis, sarcoidosis, Kyrle disease, and other perforating dermatosis.8,15,26 The majority of the patients described with EPS associated with the use of D-penicillamine had used the drug for long periods, one of whom for 19 years (average duration 11.3 years). The patient in question had been using it for 11 years.12,15 EPS clinical course is unpredictable. Occasional spontaneous involution may occur, which results in atrophic scar and local hypopigmentation, mainly in cases of EPS of idiopathic type. In the case of EPS induced by D-penicillamine, the lesions are likely to be more persistent, and, only rarely does spontaneous involution occur.8,9,15,27 However, there has been a case in which regression occurred due to a decrease in the dose of D-penicillamine.8 Regarding the patient in question, there was no regression of the lesions even following suspension of the drug. The use of corticosteroid, either topical or by local infiltration, has produced unsatisfactory results. Derma abrasion, surgical exeresis and electron surgery have evolved to queloidal scars.4,8,15 Cryotherapy with liquid nitrogen is considered to be a good treatment option. 8,15,28 However, it is poorly tolerated by patients with multiple and extensive lesions.9 The authors performed two cryotherapy sessions without the satisfactory results referred to in the literature. The oral administration of isotretinoin (0.5mg/kg/day) for at least 6 weeks, with good results should be mentioned. However, some patients may present a worsening in their clinical features and also present elastic pseudo-xanthoma after long periods of treatment with this drug.9,15 In conclusion, it can be said that the main proposal of this study is to draw attention to the necessity of a periodic dermatological evaluation of those patients using D-penicillamine for long periods of time because they may present dermatosis other than EPS.
