TRATAMENTO E ACOMPANHAMENTO 32 TRATAMENTO - OBJETIVOS • Erradicar o processo infeccioso. • E prevenir recorrências. • Obs: O tratamento inicial com antibióticos é empírico observando-se a idade e o estado geral do paciente. • A via parenteral é reservada ,principalmente , a lactentes e aqueles com acometimento do estado geral e com vômitos. RECOMENDAÇÕES GERAIS •Hidratar o paciente. •Ritmo intestinal fisiologico •Micções frequentes • O fármaco deve ter sempre propriedades bactericidas. •O tratamento nunca deve ser inferior a sete dias, e preferentemente por 10 dias contínuos . •Se o tratamento inicial for por via parenteral, sugere-se reavaliar o paciente em 48 a 72 horas e, havendo melhora, pode-se optar por completar o tratamento por via oral com antibiótico de espectro semelhante ao parenteral. Sexo masculino Branco 2 anos e 10 meses Natural e procedente de Betim – MG Informante : a mãe HISTÓRIA DA MOLÉSTIA ATUAL •QP: Febre e dor ao urinar •HDA: Há 3 dias, iniciou quadro de febre de 38-39º C associado algúria. Relata no período 2 episódios de vômitos. Nega sintomas respiratórios. • Aceita parcialmente a dieta. Diurese presente. Fezes presentes e sem alterações. HISTÓRIA PREGRESSA •Mãe G3P3A0, gestação a termo, parto vaginal; fez pré-natal; sem intercorrências; chorou ao nascer; PN: 3200 g. •Vacinação completa; •Nega alergia medicamentosa; •Nega quadros anteriores de ITU; •Fazendo uso de analgésicos. HISTÓRIA FAMILIAR E PSICOSSOCIAL •HF Mãe (25 a): dona de casa, saudável; Pai (39 a): saudável; Irmã (4 a): saudável; Irmã (1 a): saudável. •HPS Reside em casa de alvenaria (5 cômodos) com a família. Saneamento básico presente. Sem animais domésticos. 21/09/2017 10 EXAME FÍSICO •Eupneico, corado, hidratado, afebril; • SNC: Ativo e reativo; sem sinais de irritação meníngea; • Orofaringe e Otoscopia: NDN; • AR: pulmões limpos; • ACV: RCR em 2T, BNF, sem sopros; • Abdome: flácido, doloroso á palpação profunda em andar inferior. Giordano: ? • Extremidades: boa perfusão; sem edema.
