MANIFESTAÇÕES DERMATOLÓGICAS NA COVID-19: REVISÃO DE LITERATURA
RESUMO:
Introdução: A COVID-19, infeção causada pelo coronavírus SARS-CoV-2, surgiu na China em meados de dezembro de 2019, em março de 2020 obtendo proporções mundiais, chegando ao patamar de pandemia. Objetivo: Essa doença face ao seu perfil, veio mudar o paradigma da medicina de forma global, sobretudo no que se refere ao seu tratamento e prognóstico devido seu poder de letalidade. Metodologia: Dessa forma, este trabalho tem o objetivo de realizar uma revisão da literatura acerca das manifestações dermatológicas da COVID-19 para melhor compreensão da doença. O presente trabalho trata-se de um estudo descritivo de revisão, tipo de pesquisa que organiza, crítica e integra as evidências disponíveis publicadas na área da saúde. Assim, utilizou-se os seguintes delineamentos: período de publicação, idiomas, bases de dados nacionais e internacionais, critério de inclusão e a utilização dos descritores (covid-19, manifestações clínicas, manifestações dermatológicas, infecções por coronavírus, Síndrome Respiratória Aguda Grave). Foram evidenciadas três categorias temáticas: estudos que abordavam aspectos referentes ao vírus, outros que analisam a manifestação clínica da doença e por fim trabalhos que apontavam os aspectos dermatológicos da patologia. Conclusão: Observa-se, portanto, que o novo coronavírus está também acometendo pessoas mais jovens. Com relação às manifestações clínicas, de acordo com a literaturas os sinais clínicos surgem após as primeiras semanas, podendo então surgir manifestações cutâneas, como as de áreas acrais eritemato-edematosas encimadas por algumas vesículas ou pústulas, erupções vesiculares, geralmente monomórficas e dispostas no tronco, lesões urticariformes e a exantema ou lesões maculopapulares e livedo ou necrose, com diferentes graus de doença vascular oclusiva nos pacientes, em que, para o fechamento do diagnóstico, careciam de fotografias clínicas de qualidade e de caracterização histopatológica. Esse conhecimento é importante pois permite maior entendimento desta doença, sendo uma ferramenta útil e sinalizadora no diagnóstico de pacientes oligossintomáticos, sendo importante no controle epidemiológico, principalmente em áreas onde os exames diagnósticos são escassos. Portanto, o entendimento acerca das manifestações cutâneas da COVID-19 até o momento mostra-se extremamente útil para auxiliar na suspeita clínica precoce da doença, principalmente no seu reconhecimento em pacientes oligossintomáticos.
ABSTRACT:
Introduction: The COVID-19, infection caused by the SARS-CoV-2 coronavirus, appeared in China in mid-December 2019, in March 2020 obtaining worldwide proportions, reaching the level of pandemic. Objective: This disease has changed the paradigm of medicine globally, especially with regard to its treatment and prognosis due to its power of lethality. Methods: This work aims to review the literature on dermatological manifestations of COVID-19 for a better understanding of the disease. The present work is a descriptive review study, a type of research that organizes, criticizes and integrates the available evidence published in the health area. Thus, the following outlines were used: period of publication, languages, national and international databases, inclusion criteria and the use of descriptors (covid-19, clinical manifestations, dermatological manifestations, infections by coronavirus, Severe Acute Respiratory Syndrome). Three thematic categories were evidenced: studies that approached aspects related to the virus, others that analyzed the clinical manifestation of the disease and finally works that pointed out the dermatological aspects of the pathology. Conclusion: It is observed, therefore, that the new coronavirus is also affecting younger people. Regarding the clinical manifestations, according to the literature, the clinical signs appear after the first weeks, and then skin manifestations may appear, such as those of erythemato-edematous acral areas topped by some vesicles or pustules, vesicular eruptions, Generally monomorphic and arranged in the trunk, urticariform lesions and the exanthema or maculopapular lesions and livedo or necrosis, with different degrees of occlusive vascular disease in the patients, where for the closing of the diagnosis, they lacked clinical photographs of quality and histopathological characterization. This knowledge is important because it allows a better understanding of this disease, being a useful and signaling tool in the diagnosis of oligosymptomatic patients, being important in epidemiological control, especially in areas where diagnostic tests are scarce. Therefore, the understanding about the cutaneous manifestations of COVID-19 so far is extremely useful to assist in the early clinical suspicion of the disease, especially in its recognition in oligosymptomatic patients.
