The accident report aims to notify all accidents or relapses of accidents that involve the absence of the worker from the workplace for at least one day, except for the day the accident occurred, prior to medical leave.
The accident report must be completed by the person or administrative unit designated by the company's management, extracting the information from the unit where the accident occurred and from whoever provided the health care (data from the Medical Leave Report).
The accident report must be delivered to the corresponding Mutual Accident and Occupational Disease Insurance Company, the General Directorate of Computer Science and Statistics of the Ministry of Labor and Social Security, the labor authority, the employer, and the worker.
