These can be very fine in appearance. Therefore, magnified exam- ination under cobalt blue light, follow- ing application of topical fluorescein to the ocular surface, is recommended. Rose Bengal stain may also be used to aid identification of dendritic ulcers (Fig 3). However, it can be locally irritant so the ocular surface should be flushed thoroughly following its application. Geographic corneal ulceration Larger areas of geographic corneal ulceration may also develop as a result of primary infection.6 These may be single or multiple (Fig 4) in appearance. In recrudescent infections, either dendritic or geographic corneal ulceration may be a clinical feature. Chronic stromal keratitis Following multiple bouts of recrudescent disease or periods of chronic ulceration, the corneal stroma may develop chronic inflam- matory changes including neovascularisation, inflammatory cell infiltration, pigmentation, scarring and fibrosis (Fig 5). This chronic stromal keratitis is a result of an (ineffective) immune response to viral antigens sequestered within the cornea.17 Symblepharon Severe conjunctivitis in kittens and adolescent cats may lead to adhesion of the conjunctiva to itself (or to the cornea if corneal ulceration has been present).4 Such symblepharon for- mation can cause significant ocular problems including inability to blink, destruction of the lacrimal gland ductules (with resultant func- tional keratoconjunctivitis sicca [KCS]), and conjunctivalisation of the cornea, leading to blindness (Fig 6). Lesão inflamatória. Descrição macroscópica: recebidos múltiplos fragmentos diminutos sem orientação tecidual, de superfície irregular, coloração acastanhada e consistência friável. Descrição histológica: mucosa nasal: em submucosa há infiltrado inflamatório difuso acentuado composto por linfócitos, plasmócitos e ocasionais macrófagos e neutrófilos associado à discreta deposição de material fibrilar eosinofílico (fibrina) e proliferação de tecido conjuntivo fibrovascular. Não foram observadas células neoplásicas ou agentes infecciosos na amostra analisada. Diagnóstico: Rinite linfoplasmocítica crônica. Vinte e um (56,8%) tinham RSCI, nove (24,3%) neoplasias, três (8,1%) rinites fúngicas [Cryptococose (2), Sporotricose (1)], três (8,1%) alterações estruturais [corpo estranho (1), fístula oronasal (1) e palato mole alongado (1)] e um (2,7%) pólipo nasofaríngeo. Os sinais clínicos mais comuns foram espirros (94,6%), secreção nasal (91,9%) e estertor respiratório (89,2%). As radiografias revelaram alterações de opacificação da cavidade nasal (93,9%), principalmentebilateral (66,7%), perda de definição dos ossos turbinados (84,8%) e envolvimento de seios frontais (72,7%), o qual ocorreu mais bilateralmente (66,7%). As associações estatísticas foram significativas entre os gatos com RSCI e o estilo de vida multi-cat, secreção nasal mucopurulenta e presença de opacificação bilateral de seios frontais nas radiografias. Os 5 gatos diagnosticados com outras doenças, que não a RSCI, tiveram associação significativa com diminuição de apetite, secreção nasal sanguinolenta, deformidades faciais e desvio de septo nas radiografias. A PCR para Mycoplasma spp. foi positiva em apenas um caso de RSCI e todos os gatos com RSCI tiveram resultados negativos para a análise imuno-histoquímica para FHV-1 e FCV. Five ‘pillars’ framework: The authorship panel has organized the Guidelines around five primary concepts (‘pillars’) that provide the frameworkfor a healthy feline environment. Understanding these principles and the unique environmental needs of the cat will help veterinarians, cat owners and care-givers to reduce stress, the incidence of stress-related disorders, and unwanted behavior in their feline patients and pets. Veterinarians have the privilege and responsi- bility to improve the health and wellbeing of cats. Several dis- eases as well as unwanted feline behaviors have been associated with stressful environ- mental situations.1–3 Providing an appropriate environment for feline patients in their home and at the veterinary practice can prevent, improve or resolve these problems.4,5 The terms environmental enrichment and environmental modifications have been used extensively in the literature to refer to envi- ronmental changes for the benefit of the cat. These terms are not used in these Guidelines because it is more accurate to focus on the cat’s environmental needs. Some strains have a predilection for the lung, and others have been found in the macrophages within the synovial membrane of joints. 21,183 Virus may also be found in visceral tissues and feces and occasionally in the urine. Oral ulcers are the most prominent patho- logic feature of FCV infection. 141 # ese ulcers begin as vesicles, which subsequently rupture, with necrosis of the overlying epithelium and in! ltration of neutrophils at the periphery and the base. Healing takes place over 2 to 3 weeks. Pulmonary lesions appear to result from an initial focal alveolitis, which leads to areas of acute exudative pneumonia and then to the development of a proliferative, interstitial pneumonia. Although primary interstitial pneumonia can occur in FCV infection, particu- larly with more virulent strains, it has probably been overemphasized in the past as a result of experimental studies using aerosol challenge rather than the more natural oronasal routes. Epidemiologic evidence suggests that a carrier state may exist in cats with Bordetella infection, with 7% to 9% of clinically healthy cats shedding the organism. 6,112 Experimental work has shown that B. bronchiseptica is shed in both oropharyngeal and nasal secretions, in some cases for at least 19 weeks a$ er infection. 15 In the same study, B. bronchisepticawas detected in two seropositive queens a$ er partu- rition despite being negative beforehand, suggesting the stress of par- turition may have initiated detectable shedding. PATHOGENESIS Feline Herpesvirus-1 # e natural routes of infection for FHV-1 are nasal, oral, and con- junctival, and viral replication takes place predominantly in the mucosae of the nasal septum, turbinates, nasopharynx, and tonsils. Viral shedding can be detected in oropharyngeal and nasal swabs as early as 24 hours a$ er infection and generally persists for 1 to 3 weeks. Viremia is rare because virus replication is normally restricted to areas of lower body temperature, such as the respiratory tract. Methods Oral mucosal biopsies, blood and swabs were collected from cats presenting with oral lesions. The tissue sections were submitted for histopathology and immunohistochemical analysis for feline calicivirus (FCV), feline leukemia virus (FeLV) and feline immunodeficiency virus (FIV). The swabs were subjected to PCR analysis for FCV, and blood for FeLV and FIV. Results The main clinical findings were dysphagia (83.3%), halitosis (72.2%), sialorrhea (44.4%), weight loss (38.8%), intense oral discomfort (33.3%), oral hemorrhage (22.2%), lackluster and fragile coat (11.1%), and dyspnea (5.5%). Gross inspection revealed bilateral lesions across the palatoglossal fold to the lateral tongue base. The lesions were diffuse, proliferative, intensely red and friable, and bled easily upon examination in 77.8% of cases. In 22.2% of cases, the lesions were multifocal to coalescent, at times forming multiple vesicles on a reddened, edematous palatoglossal fold. Microscopic examination showed that 14.8% of lesions had moderate (grade 2) and 85.2% had severe (grade 3) inflammation. Immunohistochemistry revealed the presence of FeLV antigens in the epithelium and the inflammatory infiltrate of 29.6% of the cats with FCG. FIV antigens were identified in the inflammatory infiltrate of one cat. FCV antigens were not detected in the FCG lesions. Conclusions and relevance The FCG cases analyzed could not be correlated with FCV. It is possible that FeLV plays a role as causal agent of lesions in cases where the presence of the virus has been confirmed by immunohistochemistry in epithelial samples. Mast cells were located mainly in the submucosa, although intraepithelial mast cell infiltrates were observed in 73.1% (19/26) of animals. IHC analysis Anti-FeLV and anti-FIV IHC results are shown in Table 2. Marked anti-FeLV immunostaining was noticed in the cytoplasm of epithelial cells (often around the cyto- plasm), in the nucleus and cytoplasm of the lymphoplas- macytic infiltrate, and in occasional macrophages in 30.8% (8/26) of animals (Figure 2c,d). No anti-FCV and anti-FIV immunostaining were observed in the oral lesion samples analyzed. FIV, FeLV and FCV PCR FIV and FeLV PCR results are described in Table 2. Fifteen percent (4/26) of the cases were positive for FIV, and 34.6% (9/26) were positive for FeLV. Co-infection was diagnosed in two animals. No FCV amplification prod- ucts were obtained in the 26 oral swab samples analyzed. Discussion The clinical and pathological findings observed in the cats examined in the present study were similar to those reported in previous research on FCG.1–7,9,13,19 In the present study, male cats were more often affected by FCG, although the condition does not have any gender predisposition.1,2,20 One of the possible expla- nations for the higher incidence of FCG in male cats lies in the more aggressive behavior of these animals com- pared with females, which increases the possibility of contact with infectious agents such as FIV and FeLV.21 As reported in other studies, FCG has been diagnosed in cats of all age groups.1–3,7,9,13 The samples examined in the present study were collected from adult cats (mean age 8.8 years).22 A recent study showed that older cats as well as younger cats (1–2 years of age) have more severe lesions.9 FCG is believed to affect young cats around the Figure 1 Feline oral cavity. Feline chronic gingivostomatitis is considered to have a multifactorial etiology, with viral,5,8 bacterial,6,9 and immune5 factors suggested as causal. Conditions such as periodontitis and FRLs have also been suggested to play a role in FCGS.2 Diagnosis can be made on the ba- sis of clinical findings, but determining the full extent of the dental lesions requires probing and full-mouth dental radiography.10 The diagnosis of FCGS may be confirmed with histologic analysis of affected tissues, typically re- vealing a lymphoplasmacytic infiltrate in the mucosa and submucosa.5 Although the soft tissue aspects of FCGS Dental radiographic findings in cats with chronic gingivostomatitis (2002–2012) Nicodin Farcas, DVM; Milinda J. Animals—101 cats with FCGS (cases) and 101 cats with other oral diseases (controls). Procedures—Controls were age- and treatment date–matched with cases. Conventional full-mouth dental radiographic views were evaluated for distribution, pattern, and severity of alveolar bone loss (periodontitis), tooth resorption, buccal bone expansion, tooth fractures, and retained roots. Results—All cases and 77 (76%) controls had periodontitis; differences in extent and se- verity of periodontitis were significant, with semigeneralized or generalized and moderate or severe periodontitis in 78 (77%) and 93 (92%) cases, respectively, and 28 (28%) and 38 (38%) controls, respectively. The pattern of alveolar bone loss in cases was dominated by horizontal bone loss, with a nonsignificant increase in vertical bone loss, compared with that of controls. Cases were more likely than controls to have external inflammatory root resorption (49 [49%] vs 25 [25%]) and retained roots (57 [56%] vs 28 [28%]). Fewer dental fractures occurred in cases (14 [14%]) than in controls (35 [35%]). There were no differ- ences between cases and controls in breed, sex, or presence of feline resorptive lesions or buccal bone expansion. Conclusions and Clinical Relevance—Results suggested that FCGS was associated with more widely distributed and severe periodontitis, with a higher prevalence of external in- flammatory root resorption and retained roots than other oral diseases. Full-mouth radio- graphic views are indicated for cats with FCGS to diagnose the extent of associated peri- odontitis, reveal external inflammatory root resorption, and identify retained roots. (J Am Vet Med Assoc 2014;244:339–345) are well characterized, limited data are available regard- ing the degree and extent of hard tissue involvement. The veterinary literature contains no system- atic, controlled investigations that include evaluation of dental radiographs of a large number of cats with FCGS. It has been reported that only cats severely af- fected by FCGS have periodontal disease, whereas most have no dental disease11; there is no indication whether this claim was based on probing and dental radiogra- phy or solely on clinical appearance. Other investiga- tors4 provide a vague description of the degree of dental disease in cats with FCGS and report that few patients studied required and underwent dental treatment. Feline chronic gingivostomatitis is considered to have a multifactorial etiology, with viral,5,8 bacterial,6,9 and immune5 factors suggested as causal. Conditions such as periodontitis and FRLs have also been suggested to play a role in FCGS.2 Diagnosis can be made on the ba- sis of clinical findings, but determining the full extent of the dental lesions requires probing and full-mouth dental radiography.10 The diagnosis of FCGS may be confirmed with histologic analysis of affected tissues, typically re- vealing a lymphoplasmacytic infiltrate in the mucosa and submucosa.5 Although the soft tissue aspects of FCGS Dental radiographic findings in cats with chronic gingivostomatitis (2002–2012) Nicodin Farcas, DVM; Milinda J. Verstraete, DrMedVet, MMedVet Objective—To compare dental radiographic findings in cats with and without feline chronic gingivostomatitis (FCGS). Design—Retrospective case-control study. Animals—101 cats with FCGS (cases) and 101 cats with other oral diseases (controls). Procedures—Controls were age- and treatment date–matched with cases. Conventional full-mouth dental radiographic views were evaluated for distribution, pattern, and severity of alveolar bone loss (periodontitis), tooth resorption, buccal bone expansion, tooth fractures, and retained roots. Results—All cases and 77 (76%) controls had periodontitis; differences in extent and se- verity of periodontitis were significant, with semigeneralized or generalized and moderate or severe periodontitis in 78 (77%) and 93 (92%) cases, respectively, and 28 (28%) and 38 (38%) controls, respectively. The pattern of alveolar bone loss in cases was dominated by horizontal bone loss, with a nonsignificant increase in vertical bone loss, compared with that of controls. Cases were more likely than controls to have external inflammatory root resorption (49 [49%] vs 25 [25%]) and retained roots (57 [56%] vs 28 [28%]). Fewer dental fractures occurred in cases (14 [14%]) than in controls (35 [35%]). There were no differ- ences between cases and controls in breed, sex, or presence of feline resorptive lesions or buccal bone expansion. Conclusions and Clinical Relevance—Results suggested that FCGS was associated with more widely distributed and severe periodontitis, with a higher prevalence of external in- flammatory root resorption and retained roots than other oral diseases. Full-mouth radio- graphic views are indicated for cats with FCGS to diagnose the extent of associated peri- odontitis, reveal external inflammatory root resorption, and identify retained roots.
