In all cases, try to keep the patient at rest in a quiet and silent environment (observation beds).
In cases where the pain lasts less than 72 hours: I. administer parenteral antiemetic if vomiting or previous use of medications: dimenhydrinate 30mg intravenously (IV) diluted in 100ml of 0.9% saline or dimenhydrinate 50 mg intramuscularly (IM); II. fasting, with fluid replacement (0.9% saline) intravenously (IV) if there are signs of dehydration; III. dipyrone 1 gram (2 ml) IV diluted in distilled water (8ml); III. Ketoprofen 100mg IV diluted in 0.9% saline (100ml) or 100mg IM. IV. reassess the patient in 1 hour; if there is no improvement, prescribe sumatriptan 6mg subcutaneously (1 syringe with 0.5ml), repeating the dose, if necessary, in 2 hours.
If there is improvement in pain, release the patient and refer for outpatient follow-up at the UBS.
If there is worsening or no improvement in pain after 2 hours of observation, refer the patient to a tertiary hospital through the Regulation Center for evaluation by a neurologist.
In cases where the patient arrives at the emergency department with pain for more than 72 hours (migrainous state): I. fasting and venous access for fluid replacement if necessary. Infuse 0.9% saline 500ml in an open clamp; II. Treatment of the crisis as mentioned above associated with dexamethasone 10mg (ampoule 10mg/2.5ml) IV slowly. III. If it does not improve with dexamethasone and is in the emergency department of the UBS, refer the patient to an Emergency Care Unit (UPA), where chlorpromazine 0.1-0.25mg/kg IM (ampoule 25 mg/5 ml) should be prescribed; maintain infusion of 0.9% saline; III. if the patient maintains pain after 1 hour, repeat chlorpromazine up to three times, at most.
Pay attention to arterial hypotension and signs/symptoms.

As diretrizes de manejo clínico para enxaqueca incluem manter o paciente em repouso em um ambiente tranquilo e silencioso (leitos de observação). Em casos em que a dor dura menos de 72 horas, deve-se administrar antiemético parenteral se houver vômitos ou uso prévio de medicamentos, jejum com reposição de líquidos (solução salina a 0,9%) por via intravenosa (IV) se houver sinais de desidratação, dipirona 1 grama (2 ml) IV diluída em água destilada (8 ml), cetoprofeno 100 mg IV diluído em solução salina a 0,9% (100 ml) ou 100 mg IM. Reavaliar o paciente em 1 hora; se não houver melhora, prescrever sumatriptano 6 mg subcutâneo (1 seringa com 0,5 ml), repetindo a dose, se necessário, em 2 horas. Se houver melhora na dor, liberar o paciente e encaminhá-lo para acompanhamento ambulatorial na UBS. Se houver piora ou nenhuma melhora na dor após 2 horas de observação, encaminhar o paciente para um hospital terciário através do Centro de Regulação para avaliação por um neurologista. Em casos em que o paciente chega ao pronto-socorro com dor por mais de 72 horas (estado migranoso), deve-se fazer jejum e acesso venoso para reposição de líquidos, se necessário. Infundir solução salina a 0,9% 500 ml em uma pinça aberta; Tratamento da crise conforme mencionado acima associado à dexametasona 10 mg (ampola 10 mg/2,5 ml) IV lentamente. Se não melhorar com dexametasona e estiver no pronto-socorro da UBS, encaminhar o paciente para uma Unidade de Pronto Atendimento (UPA), onde deve ser prescrito clorpromazina 0,1-0,25 mg/kg IM (ampola 25 mg/5 ml); manter infusão de solução salina a 0,9%; se o paciente mantiver a dor após 1 hora, repetir a clorpromazina até três vezes, no máximo. Deve-se prestar atenção à hipotensão arterial e aos sinais/sintomas.
