Material and methods: Fifteen knees from male cadavers with a mean age of 36 years were dissected. The ligaments were removed, with the preservation of femo- ral and tibial insertions. A digital histological analysis sys- tem was used, employing the S100 antiprotein immunohis- tochemical technique for structure counting and recognition. Freeman and Wyke’s classification was used, which consid- ers the size and the morphological aspects of the nerve end- ings, which divides them into four types. Results: 1) A statis- tically significant difference was found between the types of ACL and PCL mechanoreceptors (< 0.001); 2) no statistically significant difference was found regarding the number of mechanoreceptors between the ACL and the PCL (p = 0.245); 3) there is no significant difference regarding age and total number of receptors within anterior and posterior cruciate Rev Bras Ortop _ Vol. 40, No 9 – Setembro, 2005 535 MECANORRECEPTORES DOS LIGAMENTOS CRUZADOS DO JOELHO bém quanto ao número total de receptores nos ligamentos cruzado anterior e posterior (p = 0,790). Conclusions: 1) There was a predom- inance of types II and IV mechanoreceptors within anterior and posterior cruciate ligaments, while types I and III were less frequent; 2) There was no statistically significant differ- ence between the anterior cruciate ligament and the posteri- or cruciate ligament regarding the total number of mechan- oreceptors; 3) There was no relationship found between total number of mechanoreceptors within the anterior and poste- rior cruciate ligaments, and the age. Keywords – Knee; cruciate ligaments; mechanoreceptors; histology; cadaverINTRODUCTION Cruciate ligaments are responsible for the intrinsic stabil- ity of the knee joint and their biomechanical function has been widely studied(1). Microscopically, the ligaments are made of parallel fascicles from regularly-oriented collagen fibers, ves- sels and specialized nerve structures, known as mechanore- ceptors(2). These convert the physical energy from the tension- ing stimulus into a nervous impulse, which will be interpreted by the central nervous system, controlling the muscle tone and the reflexes, informing the positioning and the articular motion, providing the relative somatic sensation of static and dynamic position(3,4). Despite studied for a long time, still little is known about proprioception. Lephart et al have defined proprioception as the sensory ability that follows the motions (synesthesia) and the joint position (sense of articular position), determined by the mechanoreceptors of quick and slow action(5). Those nerve endings have already been demonstrated in stud- ies on knee, shoulder, ankle, cervical spine in humans and oth- er animal species, usually with techniques employing hema- toxylin-eosin and gold hydrochloride as histological stains(6). Authors demonstrated the presence of mechanoreceptors in knee cruciate ligaments(3,4,6-9). However, there are still contro- versies regarding the frequency and neurophysiologic classifi- cation. Technically limited histological methods and interpre- tation of the structures, according to morphological criteria, are the likely causes of disagreement among several authors. The purpose of this study is to compare, using an immuno- histochemical histological technique, the presence and the number of mechanoreceptors within the anterior and posteri- or cruciate ligaments (ACL and PCL) from human cadavers. We have also tried to correlate our findings with age and re- ceptor types, following the Freeman and Wyke criteria(4). C.T. MORENO, R.L. CARVALHO, J.L. COLLEONI, C. SCAPULATEMPO NO, M.T.S. MATERIAL AND METHODS The mechanoreceptors of the ante- rior and posterior cruciate ligaments of 15 cadavers with a mean age of 36 years, ranging from 17 to 64 years (SD = 16 years), were evaluated, from necropsies performed at the coroner’s office from Universidade Federal de São Paulo-Escola Paulista de Medici- na (Unifesp-EPM), after previous ap- proval by the Institutional Review Board. As a rule, studies are performed in diseased knees, having osteoarthrosis or ligament injuries(3,8,10-12). There is not any normality standard defined for a healthy ligament. For that reason, we considered the presence of any exter- nal apparent knee injuries and macro- scopic evidences of ligament and de- generative injuries as exclusion criteria, thus selecting 15 from 26 examined knees. We have not found any data dem- onstrating or suggesting the need for a comparative study concerning the sex and the affected side. Thus, in order to standardize our research, we employed only right side knees of male cadavers as samples of our material. Knee dissection for anterior and posterior cruciate ligaments removal was performed through an anterior midline longitudinal approach of ap- proximately 25 cm, with medial open- ing of extensor apparatus and lateral dislocation of the patella (figure 1). Ligaments were identified (figure 2) and detached from their femoral and tibial portions with a scalpel blade, and then immersed in 10% formaldehyde to be later sent to pathology laborato- ry for paraffin block inclusion. Os receptores The immunohistochemical method of strepto-avidin-biotin- peroxidase for the S-100 antiprotein polyclonal antibody was employed as histological stain. The mechanoreceptors were evaluated by the Freeman and Wyke’s classification, which considers the morphological as- pects and the size of the nerve endings (figure 4). Type I re- ceptors, called Ruffini endings, are globular or encapsulated and egg-shaped, having a slow adaptive function. Type II re- ceptors, called Pacini corpuscles, are elongated and conical- shaped, being responsible for a quick adaptation. Type III receptors, or Golgi endings, are large and fusiform, and are responsible for the extremely slow adaptation mechanism. Type IV receptors are relatively undifferentiated, non-corpuscular endings, made up of amyelinic nerve filaments, and can be divided into two categories: free nerve endings, responsible for pain transmission, and the amyelinic efferent endings, re- sponsible for vasomotor innervations(4). CARVALHO, J.L. COLLEONI, C. SCAPULATEMPO NO, M.T.S. Os receptores do tipo II, também conhecidos como corpúsculos de Pacini, atuam como mecanorreceptores Correlation between total number of mechanoreceptors and age No statistically significant correlation was found between cadaver age and total number of ACL mechanoreceptors (rs = 0.08, p = 0.790) using Spearman’s rank correlation coeffi- cient, as demonstrated in graph 1. No statistically significant correlation was found between cadaver age and total number of PCL mechanoreceptors (rs = 0.10, p = 0.723) using Spearman’s rank correlation coeffi- cient, as demonstrated in graph 2. DISCUSSION The correlation between proprioception and the presence of mechanoreceptors within joint tissues of the knee has been studied in animals and men. The existence of mechanorecep- tors within cruciate ligaments of the human knee is known ever since the studies by Schultz et al. There are also referenc- es regarding joint capsule, medial collateral ligament, and menisci(3,4,13). However, there is no agreement on number and types of mechanoreceptors found within the anterior and the posterior cruciate ligaments(7,10,13). In our study, no statistically significant correlation was found between the cadaver age and the total number of mech- anoreceptors. We did not find any studies comparing such variables of cruciate ligaments from normal knees. Skinner et al, while evaluating the sensation of position of the knee joint from 29 patients with ages ranging from 20 to 82 years, found that proprioception had deteriorated with increased age(14). When evaluating 29 women without previous knee injuries, Kaplan et al found a decrease in the proprioception of the knee joint in patients above 60 years and an increase below 30 years(15). The proprioception seems to decrease with the age progression, physical effort degree and fatigue(16). Using the Freeman and Wyke’s(4) classification to identify the mechanoreceptors, we could recognize, which made the clus- tering easier, and then to quantify and to compare the receptors found. Histological techniques that employ stains as hematox- ylin-eosin(17), silver(18), hydrochloride gold(3,7) and immunohis- tochemistry were applied for mechanoreceptor recognition. Our choice for the immunohistochemistry technique, with the S-100 antiprotein, is based on the work of Del Valle et al, who, study- ing the posterior cruciate ligament of human knees, found a higher sensibility of those techniques in identifying receptors(8). Using immunohistochemistry, we noticed that nerve endings can be well differentiated from blood vessels, making their iden- tification easier. This was also noted by other authors(6,8,16,19,20). Rev Bras Ortop _ Vol. 40, No 9 – Setembro, 2005 541 MECANORRECEPTORES DOS LIGAMENTOS CRUZADOS DO JOELHO For slide interpretation, we used a digital image analysis system, which allowed easier mechanoreceptor recognition due to the possibility of increased size on the computer screen, and thus, yielding an accurate measurement. We noticed in this study that in both the anterior cruciate ligament and the posterior cruciate ligament there is a predominance of recep- tors from types II and IV. The type II receptors, also known as Pacini corpuscles, act as dynamic mechanoreceptors, remain- ing inactive upon joint immobility, and activate during accel- eration and deceleration motions, having as main features a quick adaptation and a low profile of response to mechanical stress(4). Type IV was the second most frequent group in our series. It comprises the nociceptive joint system, which acti- vates when the joint is submitted to an abnormal mechanical deformation or under the effect of inflammatory mediators such as histamine, bradykinin and prostaglandins(4). The types I and III mechanoreceptors, responsible for the slow adapta- tion and having low mechanical profile, have been found in lesser quantities within both ligaments. Studying the anterior cruciate ligaments of six cadavers extracted during their necropsy, Schutte et al, using the histo- logical technique of hydrochloride gold, found three morpho- logical types of mechanoreceptors: Ruffini- and Pacini-type corpuscles and, comprising approximately 1% of the liga- ment area, free nerve endings(3). Pacini corpuscles were the most frequent in that study. Katonis et al, studying the PCL from 10 fresh cadavers and using the same histological tech- nique, obtained similar results(7). Comparing anterior and posterior cruciate ligaments of adult sheep, regarding the total number and type of mechan- oreceptors, Raunest et al didnot find statistically significant differences between those two ligaments using hydrochloride gold as an histological stain; however, besides other mechan- oreceptors, they found a higher number of Pacinian nerve end- ings(13). Despite the use of another technique and species as sample, the purpose and results from the research by Raunest et al were similar to those in our study(13). The clinical importance of the study from the microscopic neural pattern of the knee cruciate ligaments suggests, with the results obtained, that these ligaments do not only func- tion as biomechanically stabilizing structures, but also as organs responsible for the proprioception. The presence of higher quantity of dynamic mechanoreceptors, which acti- vate during acceleration and deceleration motions, creates some concern regarding ligament injuries and their different therapeutic modalities, because it is known that, after cruci- ate ligaments injury, there is an important deficit in knee prop- dinâmicos, permanecem inativos na articulação imóvel, ativos durante os movimentos de aceleração e desaceleração e têm como características a adaptação rápida e baixo perfil de resposta ao estresse mecânico(4). 3) Não foi encontrada relação entre o número total de me- canorreceptores e a idade nos ligamentos cruzados anterior e posterior. rioception, which is related to the decrease in nerve endings number. Following this line of research, which has been trying to demonstrate the normal microscopic pattern of knee cruciate ligaments innervation, we intend to correlate those data, in the future, with certain injuries and their clinical and surgi- cal implications. CONCLUSIONS 1) There was no statistically significant difference between the anterior cruciate ligament and the posterior cruciate lig- ament regarding the total number of mechanoreceptors. 2) There was a predominance of types II and IV mechan- oreceptors, while types I and III were the less frequent within the anterior and posterior ligaments. 3) No relationship was found between the total number of mechanoreceptors and the age in anterior and posterior cru- ciate ligaments.
