The common inherited folate gene mutation, MTHFR 677 C-->T , which greatly impairs folate metabolism, has recently been found in association with cervical and endometrial neoplasms (Piyathilake CJ, 2000, Thomson SW, 2000, Kim YI, 2000, Esteller M, 1997). INFECÇÃO PELO HPV Tipos: mais de 150 ~30 tipos relacionados com o aumento do risco para carcinoma. Classificação : Alto risco: 16,18,31 e 45 Baixo risco: 6 e 11 Infecção pelo HPV HPV de baixo risco – praticamente não produzem lesão progressiva. HPV de alto risco - ~75% nunca desenvolverão lesões. Lesões intra-epiteliais escamosas: ¾ das lesões de todos os graus não progridem. A maioria das infecções pelo HPV desaparecem dentro de um a dois anos. CARACTERÍSTICAS DO HPV Vírus DNA de dupla hélice Replicação intracelular Vírions –partículas virais completas: Genoma fechado, circular com duas espirais de DNA unidas por ligação covalente com~8mil pares de bases GENOMA DO HPV: Regiões: URR(upstream regulatory region ou long control,zona larga) – 15% do total do genoma, atua na origem da replicação do DNA viral LATE (tardia) – 40% do genoma total viral (L1 e L2) – codificam proteínas do capsídeo viral EARLY (precoce)- 45% do genoma total viral ( E1,E2, E4,E5,E6 e E7) *ORFs (open reading frames, janelas de leitura aberta) – sequências de DNA envolvidas na produção de proteínas GENOMA DO HPV Genes early- são expressos logo após o início da infecção e codificam proteínas não estruturais que atuam na replicação, transcrição e transformação. E2 controla a síntese de proteínas que são produzidas pelas E6 e E7. E5 - parece estar envolvida na transformação celular. E6 e E7 –codificam proteínas capazes de induzir proliferação celular e transformação. Ciclo do HPV O HPV para completar seu ciclo- depende do metabolismo das células hospedeiras infectadas. Durante ato sexual –microerosões da mucosa –vírus entram na ZT-infecta células basais, perdem seu capsídeo- núcleo- ficando no seu estado epissomal – Infecção latente. Amadurecimento das células-desenvolvimento completo do vírus-lise e ruptura celular – liberação dos vírions. Tumour suppressor gene products have the normal task of halting cell growth. The HPV E6 gene product binds p53 and the HPV E7 gene product binds pRb. However the E6/E7 proteins of HPV 16 and HPV 18 have a much higher affinity for the tumour suppressor gene products than the E6/E7 proteins of HPV 6 and 11. This difference helps explain the difference in the transforming abilities of the virus associated with benign and malignant lesions. If the function of p53 is disrupted then the cell cycle does not pause to allow repair of damaged DNA. This results in an accumulation of mutations in the cells and it is postulated that eventually one of the proto-oncogenes will mutate. Proto-oncogenes are genes involved in normal cell growth and division. When mutated they promote uncontrolled cell growth and are known as oncogenes. In the "normal" life cycle of the virus the production of E6 and E7 are controlled by the E2 gene product. However in cancers the E2 gene is disrupted during integration of the viral genome into the host cell. This results in uncontrolled production of E6 and E7 for interaction with the tumour suppressor gene products. Uma vez que o E2 é inativado pela abertura da molecula virótica, há uma superexpressão dos genes E6 e E7. O potencial oncogênico do virus é relacionado aos produtos destes genes, que interagem e inativam proteínas celulares derivadas dos genes supressores de tumores p53 e p105-RB, ale´m de promover a degradação destes genes bloqueando a sua ação. A oncogenicidade portanto vais depender diretamente do grau de afinidade entre as proteinas dereivadas dos genes supressores de tumores e as proteínas virais derivadas de E6 e E7. These nuclear abnormalities persist throughout the epithelium irrespective of cytoplasmic maturation towards the surface. Mitotic rate is increased and abnormal mitotic figures may be seen. Patologia, CCS, UFPE 62 DIFERENCIAÇÃO DE NIC 2 E NIC 3: a displasia metaplásica (displasia moderada) é representada por células arredondadas ou ovaladas, distribuídas em lençóis.Na displasia moderada já começa a se observar grumos de cromatina. Compreende cerca de 1/8 das displasias e antecede com nuito maior frequencia o ca in situ. A taxa de progressão deste tipo de displasia para ca in situ tem sido reportada entre 30-40%.O sítio anatomico deste tipo de displasia é a porção proximal da zona de transformação. As formas metaplásicas mais maduras em direção ao orifício cervical externo e as formas mais imaturas em direção ao canal endocervical. A área nuclear relativa é de aproximadamente 32%, e os núcleos são menores que àqueles encontrados na displasia não queratinizante.Os núcleos são cerca de 2-3 vezes o tamanho de um núcleo de uma célula metaplásica normal. As células são isolada ou em lençóis.Cromatina finamente granular podendo ter cromocentros. “SIL” DE ALTO GRAU – NIC 2 CITOPLASMA CIANOFÍLICO, MAIS DENSO QUE NAS CÉLULAS DA DISPLASIA LEVE NÚCLEO OCUPA METADE A 2/3 DA ÁREA CELULAR, 2-3X O TAMANHO DO NÚCLEO DE UMA CÉL. METAPLÁSICA NORMAL ÁREA NUCLEAR: (2/3 da área celular) CROMATINA FINAMENTE GRANULAR PODENDO TER CROMOCENTROS Prof. Daisy Lima Dep. Patologia, CCS, UFPE 63 Na displasia metaplasica (NIC 2), o nucleo ocupa metade a 2/3 do citoplasma e é redondo, oval ou irregular. O citoplasma geralmente é mais denso que na displasia leve e é geralmente cianofilico. O diagnostico diferencial compreende células metaplasicas imaturas principalmente com alteraçãoes degenerativas sobrepostas. Deve-se observar a cromatina geralmente borrada e indistinta ou picnótica. Em NIC 3 o nucleo ocupa pelo menos 2/3 da area celular. área nuclear relativa é de aproximadamente 32%, e os núcleos são menores que àqueles encontrados na displasia não queratinizante.Os núcleos são cerca de 2-3 vezes o tamanho de um núcleo de uma célula metaplásica normal. Este padrão de cromatina reflete a imaturidade celular e a atividade biológica aumentada destas céls. A área nuclear das céls do carcinoma in situ varia de 32% a 75% dependendo do tipo morfológico do ca in situ. Micronucléolo é raro no ca in situ. Macronucléolo aponta para o caráter de ca invasivo. Variações na massa citoplasmática e tamanho nuclear diferencia as células dos 3 tipos de ca in situ: pequenas céls, céls intermediárias e céls grandes. Na displasia leve a cromatina é fina mostrando que a célula é biologicamente inativa. Quando o citoplasma se torna mais imaturo, ou seja quando a lesão é mais grave, a cromatina progressivamente vai aumentando os grumos, refletindo a sua natureza biologicamente ativa. Geralmente o grau de anormalidade nuclear se correlaciona bem com a proporção do epitelio mostrando maturação. PIM has been suggested to be a subset of condyloma associated with low-risk type human papilloma virus (HPV), however, the etiologic role of HPV and biologic behavior of the disease are still elusive. We compared the clinical and histopathological findings, immunohistochemical expression of Ki-67 and p53 protein, and HPV typing of 5 cases of PIM with SM (n=9), HSIL (n=6), and PSC (n=4) to know the helpful features for the differential diagnosis. Histologically, all 5 cases showed a papillary proliferation of immature metaplastic cells involving the proximal transformation zone and endocervix. On HPV typing by polymerase chain reaction-restriction fragment length polymorphism, 2 out of 5 PIM were confirmed to have HPV 6 or HPV 11, while 2 out of 4 PSC were proved having HPV 31 and HPV 16 each. Ki-67 labeling index and mitotic index of PIM were significantly lower than those of HSIL or PSC. There were no significant differences of Ki-67 labeling index and mitotic index between PIM and SM.
