The health sector reforms undertaken in the 1990s, as a thematic expression of liberal economic reforms originating from the Washington Consensus, failed. The reason for the failure is that these reforms focused on key ideas such as privatization, separation of functions, new organizational arrangements, and the introduction of competition mechanisms that were unable to generate value for ordinary citizens who use health care systems. Because of this, some speak of a third generation of health reforms that will focus on seeking equity, integrating health care systems, valuing Primary Health Care (PHC), introducing health technology assessment and evidence-based medicine, and empowering citizens (HAM, 1997). Health reforms remain necessary, but a new agenda will have to be imposed and will have as its main objectives to generate value for people who use health care systems and reduce inequities. For this, changes will have to be made through some major movements. Which of the movements below represents the direction of this new agenda?
A) From evidence-based decision-making to opinion-based decision-making. B) From fragmented systems, focused on acute conditions and events, to health care networks, focused on acute and chronic conditions. C) From financial management (clinical management) to resource management (material and financial resources). D) From a comprehensive conception of the health care system to a vision of intervention on the health problem. E) From financing based on capture and focused on individuals to financing based on volume.
A alternativa correta é a letra B) Do sistema fragmentado, focado em condições e eventos agudos, para redes de atenção à saúde, focadas em condições agudas e crônicas.
