A relação entre o tamanho das vias aéreas e o fluxo sanguíneo regional na posição ortostática é que a perfusão é reduzida nos ápices devido à força gravitacional, permitindo que os alvéolos sejam completamente expandidos, o que pode comprimir ainda mais os vasos sanguíneos, reduzindo a perfusão sanguínea. Já na base dos pulmões, a perfusão é aumentada devido à gravidade, mas os vasos sanguíneos com diâmetros maiores podem impedir a expansão completa dos alvéolos e reduzir seus diâmetros.
