Método pesquisa-intervenção de abordagem qualitativa. Participaram 30 profissionais de saúde mental de quatro Centros de Atenção Psicossocial Álcool e Drogas da região central do Brasil. Os dados foram coletados por meio de instrumentos autoaplicáveis e rodas de conversa com roteiro semiestruturado. Para a análise dos dados, utilizou-se a Análise de Conteúdo, modalidade Temática, com o auxílio dosoftware ATLAS.ti. Resultados a categoria temática Potencialidades das intervenções grupais contemplou quatro subcategorias que abordaram aspectos relacionados à estrutura física, aos recursos materiais, aos aspectos dos usuários, aos profissionais e aos processos de trabalho. Considerações finais e as implicações para a prática a potência da prática com grupos estava presente em grande parte dos atendimentos nos serviços pesquisados com a identificação de inúmeros fatores terapêuticos aos usuários. As intervenções aumentaram a fluidez dos processos de trabalho, razão pela qual necessitam ser mais difundidas e implementadas pelas equipes multidisciplinares nos serviços de saúde mental. AB - Resumen Objetivo analizar el potencial de las intervenciones grupales en los Centros de Atención Psicosocial por Alcohol y Drogas, desde la perspectiva de los profesionales. Método investigación de intervención con enfoque cualitativo. Participaron treinta profesionales de la salud mental de cuatro Centros de Atención Psicosocial por Alcohol y Drogas de la región central de Brasil. Los datos se recolectaron a través de instrumentos autoadministrados y ruedas de conversación, con un guión semiestructurado. Para el análisis de los datos se utilizó el Análisis de Contenido, modalidad temática, con la ayuda del software ATLAS.ti. Resultados la categoría temática Potenciales de intervenciones grupales incluyó cuatro subcategorías que abordaron aspectos relacionados con la estructura física, recursos materiales, aspectos de usuarios, profesionales y procesos de trabajo. Consideraciones finales e implicaciones para la práctica el poder de la práctica con grupos estuvo presente en gran parte de las consultas en los servicios encuestados, identificando numerosos factores terapéuticos para los usuarios. Las intervenciones aumentaron la fluidez de los procesos de trabajo, por lo que necesitan ser más generalizadas e implementadas por equipos multidisciplinares en los servicios de salud mental. AB - Abstract Objective to analyze the potential of group interventions in Psychosocial Care Centers for Alcohol and Drugs from the perspective of professionals. Method intervention-research of qualitative approach. Thirty mental health professionals from four Psychosocial Care Centers for Alcohol and Drugs in the central region of Brazil participated. The data was collected through self-applied instruments and conversation rounds with a semi-structured script. For data analysis, we used Content Analysis, Thematic modality, with the help of the ATLAS.ti software. Results the thematic category Potential of group interventions contemplated four subcategories that approached aspects related to the physical structure, material resources, user aspects, professionals and work processes. Final considerations and implications for practice the power of the practice with groups was present in much of the care in the services researched with the identification of numerous therapeutic factors for users. The interventions increased the fluidity of the work processes, which is why they need to be more widespread and implemented by multidisciplinary teams in mental health services. A partir da inserção semiprofissional de estudantes de graduação em Psicologia de uma faculdade privada em Fortaleza/CE em um Centro de Atenção Psicossocial Álcool e outras Drogas (CAPSad), tomou-se a sala de espera como um dispositivo interventivo e analisador do serviço. Com base na demanda do campo e nas leituras sobre clínica ampliada, buscou-se produzir um espaço de escuta e acolhimento. Ademais, o trabalho fundamentou-se na perspectiva da Redução de Riscos e Danos (RRD), no sentido de ampliar o olhar sobre a questão das drogas e possibilitar outras formas de pensar e atuar no campo da saúde mental coletiva. As temáticas problematizadas tiveram efeitos tanto para os usuários e a equipe do serviço, como para os estagiários, pois a cada encontro construía-se em ato um espaço de empoderamento e controle social. Apesar das dificuldades infraestruturais, percebemos que o CAPS, impulsionado por uma equipe implicada política e eticamente, consegue funcionar, apostando nas potencialidades da comunidade, proporcionando coletivamente outras possibilidades de existência e se afastando da perspectiva reducionista de saúde como ausência de doença. AB - This study aims to present an experience concerning of interventions made in the waiting room, pointing out its political power to strengthen the Brazilian Psychiatric Reform (RPB). From the semiprofessional insertion of undergraduate students in Psychology from a private college in Fortaleza/CE in a Center for Psychosocial Care Alcohol and Other Drugs (CAPSad), the waiting room was taken as an intervention device and analyzer of the service. Based on the demand of the field and the readings on expanded clinic, it was sought to produce a space of listening and welcoming. In addition, the work was based on the perspective of Reducing Risk and Damage (DRR), in order to broaden the view on the issue of drugs and enable other ways of thinking and acting in the field of collective mental health. The problematized themes had effects both for the users and the staff of the service, as well as for the trainees, because at each meeting an area of empowerment and social control was built. Despite the infrastructural difficulties, we realize that the CAPS, driven by a team politically and ethically Anti-Asylum Campaign, manages to work, betting on the potentialities of the community, collectively providing other possibilities of existence and moving away from the perspective reductionist as absence of disease. SETTING, DESIGN AND PATIENTS, PARTICIPANTS: Patients at risk for opioid abuse enrolled in Medicare plans were identified from July 1, 2012 to April 30, 2014 (N = 2,391), based on a published predictive model, and linked to 4,353 opioid-prescribing physicians. Patient-physician clusters were randomly assigned to one of four interventions using factorial design. INTERVENTIONS: Physicians received one of the following: Arm 1, patient information; Arm 2, links to educational materials for diagnosis and management of pain; Arm 3, both patient information and links to educational materials; or Arm 4, no communication. MAIN OUTCOME MEASURES: Difference-in-difference analyses compared opioid and pain prescriptions, chronic high-dose opioid use, uncoordinated opioid use, and opioid-related emergency department (ED) visits. Logistic regression compared diagnosis of opioid abuse between cases and controls postindex. RESULTS: Mailings had no significant impact on numbers of opioid or pain medications filled, chronic high-dose opioid use, uncoordinated opioid use, ED visits, or rate of diagnosed opioid abuse. Relative to Arm 4, odds ratios (95% CI) for diagnosed opioid abuse were Arm 1, 0.95(0.63-1.42); Arm 2, 0.83(0.55-1.27); Arm 3, 0.72(0.46-1.13). While 84.7 percent had ≥1 psychiatric diagnoses during preindex (p = 0.89 between arms), only 9.5 percent had ≥1 visit with mental health specialists (p = 0.53 between arms). CONCLUSIONS: Although this intervention did not affect pain-related outcomes, future interventions involving care coordination across primary care and mental health may impact opioid abuse and improve quality of life of patients with pain. People who inject opioids are also at disproportionate risk for contracting the human immunodeficiency virus (HIV) and hepatitis C virus (HCV). This study tests an approach that has been shown to improve recovery rates: medication along with other supportive services (medication-assisted treatment, or MAT) against MAT combined with a smartphone innovation called A-CHESS (MAT + A-CHESS). METHODS/DESIGN: This unblinded study will randomly assign 440 patients to receive MAT + A-CHESS or MAT alone. Eligible patients will meet criteria for having an opioid use disorder of at least moderate severity and will be taking methadone, injectable naltrexone, or buprenorphine. Patients with A-CHESS will have smartphones for 16 months; all patients will be followed for 24 months. The primary outcome is the difference between patients in the two arms in percentage of days using illicit opioids during the 24-month intervention. Secondary outcomes are differences between patients receiving MAT + A-CHESS versus MAT in other substance use, quality of life, retention in treatment, health service use, and, related to HIV and HCV, screening and testing rates, medication adherence, risk behaviors, and links to care. We will also examine mediators and moderators of the effects of MAT + A-CHESS. We will measure variables at baseline and months 4, 8, 12, 16, 20, and 24. At each point, patients will respond to a 20- to 30-min phone survey; urine screens will be collected at baseline and up to twice a month thereafter. We will use mixed-effects to evaluate the primary and secondary outcomes, with baseline scores functioning as covariates, treatment condition as a between-subject factor, and the outcomes reflecting scores for a given assessment at the six time points. Separate analyses will be conducted for each outcome. DISCUSSION: A-CHESS has been shown to improve recovery for people with alcohol dependence. It offers an adaptive and extensive menu of services and can attend to patients nearly as constantly as addiction does. This suggests the possibility of increasing both the effectiveness of, and access to, treatment for opioid dependence. TRIAL REGISTRATION: ClinicalTrials.gov, NCT02712034 . Para fundamentá-la, apresenta o paradigma proibicionista e sua influência na constituição das intervenções direcionadas às pessoas que usam drogas, fundamentadas prioritariamente no discurso jurídico-moral e nos dispositivos de criminalização e medicalização, e discute o paradigma da redução de danos e as possibilidades de construção de intervenções singulares e cidadãs, que inspiram a proposta de estágio em questão. A metodologia de "Escuta no pátio", orientada pelos pressupostos da atenção integral à saúde e da clínica ampliada, é apresentada como dispositivo de cuidado, que oferece uma escuta qualificada por meio de conversas individuais e coletivas não agendadas, demandadas espontaneamente pelos usuários. Conclui-se que o uso de tecnologias relacionais no cuidado em saúde tende a potencializar o exercício do acolhimento e do vínculo, o que amplia a eficácia das ações, ao favorecer a participação do usuário e seu engajamento na elaboração de estratégias de cuidado e preservação. AB - This paper presents a proposal ofa supervised internshipin Psychology carried out at a CAPS ad (Center for Psychosocial Attention, Alcohol and Drugs) named "Hearing on the patio". To substantiate this practice we present the prohibitionist paradigm and its influence on the constitution of interventions for drug users, initially grounded on the moral-legal speech and the devices of criminalization and medicalization, and then we discuss the harm reduction paradigm and the possibilities of creating individualized, humane interventions, which inspired this internship proposal. The methodology hereby adopted is oriented by the assumptions of integrated health care and expanded clinic, and presented as a care mechanism that provides a high quality hearing service through both individual and in-group unscheduled conversations, spontaneously requested by the users. We observed that the usage of relational technologies in health care tends to strengthen acceptance and bonding, amplifying the effectiveness of the actions by favoring users' participation and their engagement at the elaboration of care and preservation strategies. Para fundamentarla, se presenta el paradigma prohibicionista y su influencia en la constitución de las intervenciones direccionadas a las personas que usan drogas, basadas prioritariamente en el discurso jurídico-moral y en los dispositivos de criminalización y medicalización, y discute el paradigma de la reducción de daños y las posibilidades de construcción de intervenciones singulares y ciudadanas, que inspiran la propuesta de pasantía en cuestión. La metodología de "Escucha en el patio" orientada por los principios de la atención integral a la salud y de la clínica ampliada, es presentada como dispositivo de cuidado, que ofrece una escucha cualificada a través de conversaciones individuales y colectivas no agendadas, demandadas espontáneamente por los usuarios. Se concluye que el uso de tecnologías relacionales en el cuidado de la salud, tiende a potencializar el ejercicio del acogimiento y del vínculo, lo que amplía la eficacia de las acciones, al favorecer la participación del usuario y su compromiso en la elaboración de estrategias de cuidado y preservación. Standardized, user-friendly guidelines may have great benefit for community mental health outreach staff working with individuals with co-occurring disorders. The purpose of this study is to describe the impact of a brief, structured goal development curriculum ("Fresh Start") on clinicians' motivational interviewing skills and to assess clinician and consumer satisfaction with the curriculum. METHOD: Four clinicians, each working with 3-4 consumer participants, were assessed with the Yale Adherence and Competency Scale prior to and during use of Fresh Start to determine improvements in motivational interviewing and goal setting. Consumer participants were assessed in brief qualitative interviews to assess satisfaction. Chart-review of routinely collected substance use data was used to assess reductions in use. RESULTS: Clinicians demonstrated high ratings in relational motivational interviewing skills at baseline which were maintained at study conclusion. Clinicians had lower technical motivational interviewing skills at baseline and made modest improvements in motivational interviewing techniques targeting increasing change talk. Fresh Start participants completed an average of 10 sessions (SD = 2.35). Among the 16 consumers who volunteered to participate in using the Fresh Start curriculum, 12 finished the program and 9 decided to reduce use and composed a sobriety plan. CONCLUSIONS AND IMPLICATION FOR PRACTICE: High rates of completion indicate acceptability of the curriculum and feasibility for implementing the program in a community setting. Short, goal-setting interventions are needed and under proper conditions may be teachable in vivo. Replicating active labour market policies pursued across the globe, successive governments have used welfare-to-work programmes as mechanisms to address worklessness. Since 2008, problem drug users (PDUs) have been added to a list of groups for whom intervention is deemed necessary to encourage, enable, and sometimes coerce them into paid employment. This approach is underpinned by three beliefs relating to paid work: it sustains recovery, has a transformative potential and should be the primary duty of the responsible citizen. Using policy developments in the UK as a case study, the article explores the implications for methadone maintenance clients of connecting drug policy (premised on the belief that work is central to recovery) with welfare policy (which at present is preoccupied with reducing worklessness). METHODS: A critical analysis of policy documents, including drug strategies, Green and White papers and welfare reform legislation, alongside a review of relevant academic literature. RESULTS: The 'work first' approach which underpins current labour market activation policies in the UK and elsewhere is insufficiently flexible to accommodate the diverse needs of PDUs in recovery, and is particularly particular problematic when combined with a 'social deficit' model which concentrates on individual rather than structural barriers to employability. The use of payment-by-results mechanisms to provide employment services, coupled with the use of sanctions for those who do not engage, is likely to be particularly problematic for methadone maintenance clients. CONCLUSION: Welfare reform in the UK is likely to undermine the recovery of methadone maintenance clients. Further research is urgently needed to explore its impact on this sub-group of PDUs, alongside comparative studies to determine best practice in integrating drug and welfare policies. Parents who are dependent on psychoactive drugs are at risk of a wide range of parenting problems, and studies have found reduced sensitivity and responsiveness to both the infant's physical and emotional needs. The poor outcomes that are associated with such drug dependency appear to be linked to the multiple difficulties experienced by such parents.An increase in understanding about the crucial importance of early relationships for infant well-being has led to a focus on the development and delivery of services that are aimed at supporting parenting and parent-infant interactions. The Parents under Pressure (PuP) programme is aimed at supporting parents who are dependent on psychoactive drugs or alcohol by providing them with methods of managing their emotional regulation, and of supporting their new baby's development. An evaluation of the PuP programme in Australia with parents on methadone maintenance of children aged 3 to 8 years found significant reductions in child abuse potential, rigid parenting attitudes and child behaviour problems. METHODS/DESIGN: The study comprises a multicentre randomised controlled trial using a mixed-methods approach to data collection and analysis in order to identify which families are most able to benefit from this intervention.The study is being conducted in six family centres across the UK, and targets primary caregivers of children less than 2.5 years of age who are substance dependent. Consenting participants are randomly allocated to either the 20-week PuP programme or to standard care.The primary outcome is child abuse potential, and secondary outcomes include substance use, parental mental health and emotional regulation, parenting stress, and infant/toddler socio-emotional adjustment scale. DISCUSSION: This is one the first UK studies to examine the effectiveness of a programme targeting the parenting of substance-dependent parents of infants and toddlers, in terms of its effectiveness in improving the parent-infant relationship and reducing the potential for child abuse. Risk management philosophies emphasize the importance of swift and sure sanctions for failure to comply with program attendance requirements. In contrast, change theory emphasizes the importance of providing clients with multiple opportunities to engage in treatment. To clarify the implications of each of these philosophies, the current study examined rates of program dropout, reinstatement, and completion in a consecutive sample of 294 probation-mandated clients referred to a large batterer intervention program. Just over half (53.7%) of men completed intervention on their first attempt. Over the 2-year follow-up study period, 73 clients were reinstated once by the intervention program, 23 clients were reinstated twice, and 5 clients reinstated three (or more) times. Reinstated clients were, in general, more similar to men who failed to complete than those who completed on their first attempt. Although rates of dropout at each reentry point were quite high (56% to 80%), 32 of the 73 (43.7%) reinstated clients eventually completed. There were significant costs associated with providing clients with additional chances to complete the program, with successful reinstatement requiring an average of 7.55 phone calls to clients, 3.82 phone calls to referral agents, one letter, and 0.73 in-person meetings. Results are discussed in terms of practice and policy implications of risk management and change theory approaches to dropout. A escassez de pesquisas nesta área no contexto local, fomenta a importância da discussão sobre o uso de drogas e as formas de tratamento existentes. Essa nova possibilidade de tratamento surgida das demandas sociais do município, através de dados compilados por diversas pesquisas, que levaram o governo estadual a planejar e implementar o centro, tinha por objetivo respeitar a voluntariedade do sujeito, incentivar a construção de um ambiente humanizado, pautada na lógica da reforma psiquiátrica, no que concerne à participação da família e práticas ressocializantes. Examinamos algumas contribuições da Gestalt-terapia, Psicologia de base fenomenológica - existencial, que orienta minha linha de pesquisa e da Terapia Ocupacional, minha área de atuação, correlacionando-as como ciências e intervenções que dialogam no campo da interdisciplinaridade, tendo em vista o tratamento da drogadicção. A coleta de dados se deu através de questionários com questões abertas, respondidos por representantes da coordenação do centro, da equipe psicossocial e usuários do serviço. A perspectiva fenomenológica-existencial gestáltica configurou a fundamentação teórica na condução desse estudo, em que procuramos colaborar com este campo de investigação, desvelando alguns sentidos contidos nos diferentes discursos dos atores que trabalham ou se tratam no CCDQ e que colaboraram para configurar este relatório. Observamos a partir desses discursos, transformados em unidades de significação, que tanto o trabalho voltado às famílias, quanto as atividades ressocializantes estão deficitárias, sendo apontada também a necessidade de capacitação da equipe e a ausência de suporte da rede de serviços de saúde dentre outras questões. Pela especificidade dos resultados, pretendemos contribuir com a fundamentação de estratégias locais de ações preventivas e terapêuticas em drogadicção, além de possibilitar o conhecimento do que já vem sendo construído neste campo de atuação, tendo em vista ainda que novas ações terapêuticas podem ser construídas a partir da avaliação dos serviços já existentes prestados à população, bem como suscitar reflexões sobre as políticas, as ações e os seres humanos. AB - This research looked for to know the work developed by the Center of Cares to Chemical Dependents - C.C.D.Q., located in the municipal district of Belem, state of Para, only public service in Brazil inside of the Unique system of Health destined to offer specific treatment for chemical dependence of alcohol and drugs in internment regime in long duration, following by attendance ambulatory. The shortage of researches in this area in the local context foments the importance of the discussion about the use of drugs and the existent treatment forms. This new treatment originating from the social demands of the municipal district, through data compiled by several researches, that took the state government to implement the center, it had for objective to respect the personal desires, to motivate the construction of a humanized atmosphere, ruled in the logic of the Psychiatric Reform, in what it concerns to the participation of the family and practices re-socialization. We examined some contributions of the gestalt-therapy, Psychology of base phenomenological - existential, it guides my research line and of the Occupational therapy, my area of performance, correlating them as sciences and interventions that dialogue in the field of the interdisciplinary, with purpose of the treatment of drugs. The collection of data happened through questionnaires with open subjects, answered by representative of the coordination of the center, of the team psychosocial and users of the service. The perspective phenomenological existential gestalt configured the theoretical foundation in the transport of that study, where we tried to collaborate with this investigation field, revealing some senses contained in the actors different speeches that work or are treated in the CCDQ and that they collaborated to configure this report. We observed to leave of those speeches, transformed in units of significance, that so much the work returned to the families, as the activities re-socialization are deficient, being also pointed the need of training of the team and the absence of support of the net of services of health among other subjects. This study used within- and across-case analyses of longitudinal data from qualitative interviews with 41 dually diagnosed individuals entering residential programs to exit homelessness and receive needed services. Thematic findings include (a) "loner talk" and the need for privacy; (b) family ties as "good news, bad news"; (c) when it comes to a partner, other things come first; and (d) in search of positive people. Analyses of change in individual trajectories revealed that stronger social relationships did not coincide exactly with positive outcomes. Although positive life changes were gradual, negative changes could be precipitous. Social relationships were threatened by concentrated disadvantage, that is, a lack of social and economic currency. Findings are discussed with implications for improving services for the most vulnerable individuals who stand to benefit from the era of recovery. Etomidate has been studied mostly in adults. Our objective is to compare the efficacy of etomidate and midazolam for achieving procedural sedation and analgesia in children. METHODS: A randomized, double-blind, emergency department and orthopedic clinic-based trial was carried out among patients aged 2 to 18 years with displaced extremity fractures. Patients were administered 1 microg/kg of fentanyl and either 0.2 mg/kg of etomidate or 0.1 mg/kg of midazolam. Adequate sedation was defined, for the purpose of this study, as a score of 4 or more on the Ramsay Sedation Scale. The primary outcome was induction and recovery time. The rates of adverse events, success of fracture reduction, and parent and physician satisfaction were also compared. RESULTS: From April to August 2004, 100 of 128 eligible patients were enrolled (age 8.7+/-3.7 years; 50% male patients). A higher proportion of patients attained adequate sedation among those who received etomidate: 46 of 50 (92%) versus 18 of 50 (36%) (delta 56%; 95% confidence interval [CI] 38% to 69%). Time taken for induction (hazard ratio 4.9; 95% CI 2.2 to 10.9) and time taken for recovery (hazard ratio 2.8; 95% CI 1.5 to 5.1) were lower among patients who received etomidate. The rates of adverse events were similar in both groups, except for myoclonus and pain at the injection site, which was more frequent in the etomidate group. CONCLUSION: Induction and recovery times are shorter with etomidate compared with midazolam. At the dosages used for procedural sedation and analgesia among children with displaced extremity fracture, etomidate has higher efficacy in comparison with midazolam. Não há dúvida de que as Conferências Nacionais de Saúde e Saúde Mental têm constituído dispositivos fundamentais de participação, controle social, debate e síntese democrática das diretrizes políticas principais e de medidas operacionais nestas áreas no País. No campo da saúde mental, as Conferências têm tido um papel crucial de dar continuidade ao processo, iniciado no Brasil nos anos 70, de crítica ao modelo hospitalocêntrico de assistência, e de definir as estratégias e rumos na implementação da Reforma Psiquiátrica a partir dos anos 80, em interlocução com aspirações e experiências já em implantação em diversos países do mundo. Na I Conferência, realizada em 1987, ficou claramente nomeado o impasse do modelo centrado no hospital e predominantemente baseado no modelo médico-psiquiátrico, considerado ineficaz e oneroso para os usuários e a sociedade, violando os direitos humanos fundamentais. Se as indicações aprovadas naquela ocasião para enfrentar este desafio podem ser hoje consideradas incompletas, são ainda assim uma referência para as questões a serem enfrentadas pela Reforma Psiquiátrica Brasileira. É fundamental recordar também que esta I Conferência foi realizada no contexto dos princípios e diretrizes da 8ª Conferência Nacional de Saúde, marco histórico no campo da saúde e da construção do Sistema Único de Saúde (ver III Conferência Nacional de Saúde Mental, Caderno Informativo, 2001).
