What are the nursing care for ostomies?
Temporary or permanent intestinal outputs present exclusive elimination needs.
An individual with an ostomy uses a bag to collect effluents or an output from the stoma.
Bags are odor-proof and have a protective barrier around the stoma.
It is necessary to empty the bag when it is 1/3 to ½ full.
Change the collector bag system approximately every 3 to 7 days, depending on the individual needs of the patient.
Evaluate the color of the stoma (which should be pink or red) and the skin at each change of the collector bag.
Intestinal effluents have digestive enzymes that cause irritant dermatitis.
ILEOSTOMY
Volume of effluent drainage:
It is related to the ingested food (500 to 800 ml/day).
Gases and odors are reduced.
Characteristic of feces:
Liquid/semi-liquid, brownish-green color.
Eliminates a large amount of electrolytes and enzymes due to decreased absorption by the digestive tract.
Very alkaline pH (corrosive).
COLOSTOMY
Irrigation
Although this practice is not so common due to the improvement of odor-proof bags, some patients irrigate their sigmoid colostomies to regulate colon emptying.
A specific material designed for ostomies is used (a silicone cone connected by a plastic tube to a bag with warm water).
Collector system
An effective collector system protects the skin, contains fecal material, remains odor-free, is comfortable, and imperceptible.
The patient needs to feel secure enough to participate in any activity.
Collector bags are presented in one- and two-piece systems and are smooth or convex.
Some bags have the opening pre-cut by the manufacturer, others require the opening of the stoma.

NURSING CARE WITH OSTOMIES
After surgery, it usually takes a few days for patients with ostomies to return to normal appetite.
Patients with colostomies have no dietary restrictions as long as they maintain a healthy diet.
Discomfort.

The nursing care for ostomies includes: 1. Emptying the bag: It is necessary to empty the bag when it is 1/3 to ½ full to prevent leakage and discomfort. 2. Changing the collector bag system: The collector bag system should be changed approximately every 3 to 7 days, depending on the individual needs of the patient. This helps maintain hygiene and prevent skin irritation. 3. Evaluating the stoma and skin: At each change of the collector bag, it is important to evaluate the color of the stoma (which should be pink or red) and the skin around it. Any changes in color or skin integrity should be reported to the healthcare provider. 4. Managing skin irritation: Intestinal effluents have digestive enzymes that can cause irritant dermatitis. Proper skin care, including cleaning and applying protective barriers, is essential to prevent skin breakdown. 5. Managing ostomy output: For ileostomies, the volume of effluent drainage is related to the ingested food and can range from 500 to 800 ml per day. For colostomies, some patients may choose to irrigate their sigmoid colostomies to regulate colon emptying. This should be done using specific materials designed for ostomies. 6. Ensuring a proper collector system: An effective collector system should protect the skin, contain fecal material, remain odor-free, be comfortable, and imperceptible. The patient should feel secure enough to participate in any activity. Collector bags are available in one- and two-piece systems, with different shapes and features. 7. Monitoring stoma characteristics: It is important to observe the stoma for any changes in color, shape, size, moisture, protrusion, or mucosal integrity. Any abnormalities should be reported to the healthcare provider. 8. Educating the patient: Patients with ostomies should receive education on self-care, including proper bag management, skin care, and dietary considerations. They should also be provided with emotional support and resources to cope with the changes in their body image and lifestyle. Remember, these are general nursing care guidelines for ostomies. The specific care may vary depending on the individual's condition and healthcare provider's instructions.
