CONCEITO E CARACTERIZAÇÃO DAS NEOPLASIAS • A etiologia e progressão das neoplasias resulta do acúmulo de uma série de lesões pró-mutagênicas não-letais no DNA das células somáticas. • Em nível molecular, as neoplasias surgem de mutações em genes que regulam o crescimento celular, a apoptose ou a reparação do DNA. • Diferenciam-se das respostas adaptativas às lesões subletais por serem dotadas de capacidade de proliferação autônoma. • Apresentam graus variáveis de diferenciação podendo apresentar uma organização celular idêntica a do tecido normal correspondente ou se tornar distinta deste. • Originam-se de células com capacidade de proliferação. Imprevisível, podendo ser muito rápida, figuras de mitoses frequentes e atípicas. Diferenciação e Anaplasia Células bem diferenciadas, típicas do tecido de origem Graus variáveis de indiferenciação (pleomorfismo) com anaplasia presente e organização tecidual atípica Invasão local Arranjo compacto e bem delimitado, não invade o tecido adjacente, muitas vezes com pseudocápsula Localmente infiltrativo com invasão dos tecidos vizinhos. Pode ser expansivo. Metástase Ausente Podem estar presente caracterizando o tumor como maligno (câncer). O sufixo “oma” (tumor) é precedido pela referência à célula ou tecido de origem. NOMENCLATURA DOS TUMORES MALIGNOS Designações associadas à origem histogenética do tumor são precedidas pelo vocábulo que nomeia o tecido. DISTINÇÕES ENTRE TUMORES DISTINÇÕES DOS TUMORES Condições pré-neoplásicas ■ Condições predisponentes hereditárias - Síndromes autossômicas por perda de heterozigose do câncer hereditário (genes Rb, APC). - Síndrome do Reparo defeituoso do DNA - Cânceres Familiares (BRCA-1, BRCA2) ■ Condições predisponentes não-hereditárias - Inflamação crônica - Neoplasias benignas CONDIÇÕES PRÉ-NEOPLÁSICAS Câncer constitui-se numa doença genética cuja etiologia e progressão resulta do acúmulo de uma série de lesões pró-mutagênicas não-letais no DNA das células somáticas, lesões estas que comprometem genes associados aos programas de proliferação, sobrevivência, diferenciação e apoptose conferindo às células afetadas, capacidade de invasão e disseminação (metástase). Upstream signalling to p53 increases its level and activates its function as a transcription factor in response to a wide variety of stresses, whereas downstream components execute the appropriate cellular response. The principal sensors seem to be MDM2 and MDM4 and their interaction with p53. In non-stressed conditions these proteins bind p53, ubiquitylate it and target it for degradation by the proteasome. In stressed conditions the function of the MDM2–MDM4 complex is blocked by phosphorylation, protein-binding events and/or enhanced degradation141. Hence, phosphorylation of MDM4 is essential for the p53 response to ionizing radiation, and the response to oncogene activation depends on the binding of ARF to MDM2. Many p53-activating small molecules function by causing the release of ribosomal proteins from the nucleolus to the nucleoplasm, where they bind to MDM2 and MDM4 and inhibit their function. Molecules that activate wild-type p53 in tumours by disrupting MDM2 activity can compensate for any missing upstream components of the p53 pathway, for example the loss of ARF expression that is frequent in cancer cells142. However, defective downstream p53 signalling might substantially decrease their effectiveness. Therefore, the ability to identify tumours in which downstream p53 signalling is unaffected is important. One common feature of tyrosine kinase signaling associated with cell proliferation is regulation of glucose metabolism. In contrast to differentiated cells, proliferating cells selectively express the M2 isoform of the glycolytic enzyme pyruvate kinase (PK-M2) (9). Unlike other pyruvate kinase isoforms, PK-M2 is regulated by tyrosine-phosphorylated proteins (17). Phosphotyrosine signaling downstream of a variety of cell growth signals shares the common ability to negatively regulate PK-M2 activity (17). In response to phosphotyrosine-protein binding, PK-M2 is induced into a low-activity state. This regulation of enzyme activity may constitute a molecular switch that allows cells to metabolize glucose through glycolysis in a manner that is consistent with proliferating cell metabolism only when growth signals are present (Fig. 3). Although counterintuitive, it is the low-activity form of PK-M2 that is necessary for cell proliferation. This regulation allows PKM2 to act as a gatekeeper that dictates the flow of carbon into biosynthetic pathways versus complete catabolism for ATP production. In support of this idea, PK-M2 is required for proliferation in vivo (9).
