Fossa craniana anterior (fossa cranii anterior) • Formada pelo frontal, etmoide e esfenoide. • Assoalho consiste no frontal e forma a parede superior da órbita. No assoalho há o seio frontal. • Na região central há o etmoide. • Na linha média há uma elevação aguda (crista etmoidal), que serve de conexão para a foice do cérebro. • Em uma depressão de cada lado há as lâminas cribiformes (acomodam os bulbos olfatórios) – que são crivadas de pequenas perfurações para passagem de filamentos do nervo olfatório. 23/02/2016 2 Fossa craniana média (fossa cranii media) • Formada pelo esfenoide e temporal. • Na linha mediana, o corpo do esfenoide forma uma depressão, a fossa hipofisial, circundada por quatro protrusões ósseas, os processos clinoides. • Canal óptico: medial ao processo clinoide anterior, comunica-se com a órbita, servindo de passagem para o NC II e a. oftálmica. • Fissural orbital superior: entre a asa maior e asa menor do esfenoide, também comunica-se com a órbita. Passagem para NCII, NCIV, NC VI e divisão oftálmica de NC V. (fossa cranii media) • Forame redondo: abre-se na fossa pterigopalatina e transporata a divisão maxilar o trigêmeo. • Forame oval: transporta a divisão maxilar do NC V. • Forame espinhoso: entrada para a a. meníngea média. Fossa craniana posterior (fossa cranii posterior) • Formada pelo occipital e parte petrosa do temporal. • Na linha média há um declive íngreme liso (clivo), contínuo com o corpo do esfenoide, posterior à fossa hipofisial. Sobre ele repousa o tronco encefálico e passa pelo forame magno (que também serve de passagem para aa. Vertebrais e raiz espinal do NC XI). • Na parede desse forame há o canal do NC XII, por onde ele deixa a cavidade craniana. • Entre o occipital e a parte petrosa do temporal, há o forame jugular (passagem da v. jugular interna, NC IX, NC X e NC XI) • Na face posterior da parte petrosa há o meato acústico interno, passagem do NC VIII e NC VII. • O cerebelo repousa na fossa posterior do crânio. 23/02/2016 3 NECT • (Top)Five sequential axial NECT images from presented inferior to superior through skull base, calvarium, are depicted. Section through skull base shows major bones, sutures forming skull base. Sphenosquamosal, petrooccipital, occipitomastoid sutures are normally well seen and should not be confused with fractures. (Middle) Section through upper skull base shows anterior, middle and posterior cranial fossae as well as formation of lower vault by frontal, greater wing sphenoid, squamous temporal and occipital bones. • (Bottom) Section through lower calvarial vault showing antero-posterior linear configuration of squamosal suture, not to be confused with a fracture. Vertebrais. • Carótida interna entra na fossa média através do canal carótico, faz curvas (sifão carótico), segue anteriormente pelo seio cavernoso e superiormente na porção medial do processo clinoide anterior. • Chega à superfície do encéfalo lateralmente ao quiasma óptico. Branches.—The cervical portion of the internal carotid gives off no branches. Those from the other portions are: Ramos pré-terminais da a. carótida interna • Aa. Hipofisárias: à partir do segmento cavernoso; irrigação da neurohipófise e formação do sistema porta-hipofisário. • A. oftálmica: entra na órbita pelo canal óptico. Irriga estruturas da órbita, seio frontal e células etmoidais, parte anterior do couro cabeludo e dorso nasal. • A. corioidea anterior: irriga o trato óptico, plexo corioideo do ventrículo lateral, hipocampo e algumas estruturas profundas (p. ex. cápsula interna e globo pálido) 23/02/2016 5 Ramos pré-terminais da a. carótida interna • A. comunicante posterior: segue posteriormente e liga-se à a. cerebral posterior. • Lateralmente ao quiasma óptico, divide-se em ramos terminais: a. Cerebrais anterior e média. A. Cerebral Anterior (a. cerebri anterior) • A. cerebral anterior possui trajeto medial acima do quiasma óptico e entra na fissura longitudinal do cérebro. Une-se ao vaso contralateral por meio da a. comunicante anterior; segue contornando o joelho do corpo caloso, ramificando-se sobre a face medial dos lobos frontal e parietal, também se entende da fissura longitudinal para irrigar um trecho lateral dos lobos. A. Cerebral média (a. cerebri media) • Possui o maior território vascular cortical. Segue lateralmente desde sua origem e entra no sulco lateral, subdividindo-se e irrigando as faces superolaterais dos lobos frontal, parietal e temporal. Serve também o córtex auditivo e o lobo insular profundamente ao sulco lateral. • Os dois ramos juntos formam o ―Sistema Arterial Carótico Interno‖. A. vertebral • Origina-se da a. subclávia, sobe através dos forames transversais das vértebras cervicais, entra pelo forame magno, pela parte anterolateral do bulbo. Superiormente, converge na junção da ponte com o bulbo, formando a a. basilar. Origina aa. Espinais anterior e posterior que irrigam o bulbo e a medula. • Seu maior ramo é a. cerebelar inferior posterior que irriga a parte inferior do cerebelo. A. Basilar • Percorre a ponte, fornecendo aa. da ponte. • Origina a. cerebelar anterior inferior (irriga partes anterior e inferior do cerebelo); a. do labirinto (entra no meato acústico interno para irrigar a orelha interna). • Na junção da ponte com o mesencéfalo, forma aa. Cerebelares superiores (parte superior do mesencéfalo) e aa. Cerebrais posteriores. A. Cerebral posterior (a. cerebri posterior) • Curva-se em torno do mesencéfalo e irriga o córtex visual do lobo occipital e parte inferomedial do temporal. • Essas estruturas estão no ―Sistema Arterial Vertebrobasilar‖. 23/02/2016 6 Considerações • Os dois sistemas arteriais são unidos pelas aa. Comunicantes posteriores, que passam superoinferiormente entre as extremidades das aa. Carótidas internas com cerebrais posteriores. • Tem-se, assim, o círculo arterial do cérebro, que circunda o quiasma óptico, assoalho do hipotálamo e mesencéfalo. Círculo Arterial do Cérebro • Origina aa. Perfurantes (centrais ou ganglionares): • Aa. Perfurantes anteriores: originam-se da a. cerebral anterior, comunicantes e origem da cerebral média. Entram no encéfalo entre o quiasma óptico e o término do trato olfatório, na substância perfurada anterior. Irrigam núcleos da base, quiasma óptico, cápsula interna e hipotálamo. • Aa. Perfurantes posteriores: originam-se da aa. Cerebral posterior e comunicantes. Entram no encéfalo entre os dois pilares do cérebro (mesencéfalo), na substância perfurada posterior, irrigando a parte ventral do mesencéfalo e partes do subtálamo e hipotálamo. A. Cerebral Anterior e Comunicante Anterior Antero-lateral Ganglionic Branches • The Antero-lateral Ganglionic Branches, a group of small arteries which arise at the commencement of the middle cerebral artery, are arrangedin two sets: one, the internal striate, passes upward through the inner segments of the lentiform nucleus, and supplies it, the caudate nucleus, and the internal capsule; the other, the external striate, ascends through the outer segment of the lentiform nucleus, and supplies the caudate nucleus and the thalamus. One artery of this group is of larger size than the rest, and is of special importance, as being the artery in the brain most frequently ruptured; it has been termed by Charcot the artery of cerebral hemorrhage. It ascends between the lentiform nucleus and the external capsule, and ends in the caudate nucleus. Antero-lateral Ganglionic Branches • The inferior lateral frontal supplies the inferior frontal gyrus (Broca’s convolution) and the lateral part of the orbital surface of the frontal lobe. • The ascending frontal supplies the anterior central gyrus. • The ascending parietal is distributed to the posterior central gyrus and the lower part of the superior parietal lobule. • The parietotemporal supplies the supramarginal and angular gyri, and the posterior parts of the superior and middle temporal gyri. The temporal branches, two or three in number, are distributed to the lateral surface of the temporal lobe. • The Anterior Communicating Artery : It gives off some of the antero-medial ganglionic vessels, but these are principally derived from the anterior cerebral. • The middle cerebral artery (a. cerebri media), the largest branch of the internal carotid, runs at first lateralward in the lateral cerebral or Sylvian fissure and then backward and upward on the surface of the insula, where it divides into a number of branches which are distributed to the lateral surface of the cerebral hemisphere. 23/02/2016 7 • The posterior communicating artery : From its posterior half are given off a number of small branches, the postero-medial ganglionic branches, which, with similar vessels from the posterior cerebral, pierce the posterior perforated substance and supply the medial surface of the thalami and the walls of the third ventricle. • The posterior cerebral artery (a. cerebri posterior) : it winds around the cerebral peduncle, and reaches the tentorial surface of the occipital lobe of the cerebrum, where it breaks up into branches for the supply of the temporal and occipital lobes. 23/02/2016 8 Diagram of the arterial circulation at the base of the brain. Drenagem venosa • Composta por vv. Profundas, vv. Superficiais e seios venosos da dura-máter. • Não possuem válvulas. • VV. Cerebrais profundas drenam estruturas internas do prosencéfalo. Ressalta-se: vv. Talamoestriada e coroidea, que drenam os núcleos da base, tálamo, cápsula interna, plexo corioideo e hipocampo. Dentro de cada hemisfério fundem-se e formam a v. cerebral interna. • As duas cerebrais internas unem-se na linha média e formam a v. cerebral magna, abaixo do esplênio do corpo caloso. Esse vaso é contínuo com o seio reto, na linha mediana do tentório do cerebelo. • Vv. Superficiais estendem-se através do espaço subaracnoideo. Vv. Cerebrais superiores drenam a face superolateral dos hemisférios cerebrais e desembocam no seio sagital superior. • V. cerebral média segue ao longo do sulco lateral e desemboca no seio cavernoso. • Há dois canais anastomóticos que drenam para o seio sagital superior e o seio transverso, que são v. anastomótica superior (maior) e v. anastomótica inferior, respectivamente. • Vv. Cerebrais profundas e superficiais drenam para os seios da dura-máter, estando os maiores nas bordas fixas da foice do cérebro e tentório do cerebelo, e assoalho da cavidade craniana. 23/02/2016 10 • O seio sagital superior recebe sangue principalmente das vv. Cerebrais superiores, que ramificam sobre a face lateral dos hemisférios. A borda livre contêm o seio sagital inferior, menor, que drena vv. da face medial do hemisfério cerebral. No interior do tentório, ao longo da fixação à foice do cérebro, há o seio reto. Em seu interior passa a v. cerebral magna, que drena estruturas profundas do prosencéfalo e o seio sagital inferior. • O seio sagital superior e o seio reto convergem para a confluência dos seios, adjacente à protuberância dos seios, adjacente à protuberância occipital interna. O seio transverso é contínuo com o seio sigmoideo, que, por sua vez, une-se à v. jugular interna ao nível do forame jugular. • O seio cavernoso está lateral ao corpo do esfenoide. Recebe sangue da a. cerebral média e drena para a v. jugular interna (através do seio petroso inferior) e para o seio transverso (através do seio petroso superior). • Os dois seios cavernosos são conectados pelos seios intercavernosos, que se situam anterior e posteriormente à hipófise, formando um círculo venoso em torno dela (seio circular). Octopus Publishing Group: London, 2012. The Meninges of the Brain • The brain and medulla spinalis are enclosed within three membranes. These are named from without inward: the dura mater, the arachnoid, and the pia mater. Dura-máter • Resistente e fibrosa • Firmemente aderida à base do crânio e na linha mediana da calvária. • Forma dois grandes processos (reflexões): foice do cérebro e tentório do cerebelo. 23/02/2016 12 Aracnoide-máter • Membrana macia e translúcida, separada da dura pelo espaço subdural, por onde passam veias no trajeto para os seios venosos da dura. • Separada da pia pelo espaço subaracnoideo, que contêm uma rede filamentosa de fibrilas de tecido conjuntivo (trabéculas) e é atravessado por vasos, contêm LCR, e em alguns recessos profundos, em depressões e fissuras, formam-se cisternas subaracnoideas. Pia-máter • Microscopicamente fina, delicada e altamente vascularizada, intimamente aderida à superfície cerebral. • Subarachnoid Cisternæ (cisternæ subarachnoidales)—The cisterna cerebellomedullaris (cisterna magna) is triangular on sagittal section, and results from the arachnoid bridging over the interval between themedulla oblongata and the under surfaces of the hemispheres of the cerebellum; it is continuous with the subarachnoid cavity of the medulla spinalis at the level of the foramen magnum. • The cisterna pontis is a considerable space on the ventral aspect of the pons. It contains the basilar artery, and is continuous behind with the subarachnoid cavity of the medulla spinalis, and with the cisterna cerebellomedullaris; and in front of the pons with the cisterna interpeduncularis. 23/02/2016 13 • The cisterna interpeduncularis (cisterna basalis) is a wide cavity where the arachnoid extends across between the two temporal lobes. It encloses the cerebral peduncles and the structures contained in the interpeduncular fossa, and contains the arterial circle of Willis. In front, the cisterna interpeduncularis extends forward across the optic chiasma, forming the cisterna chiasmatis, and on to the upper surface of the corpus callosum, for the arachnoid stretches across from one cerebral hemisphere to the other immediately beneath the free border of the falx cerebri, and thus leaves a space in which the anterior cerebral arteries are contained. • The cisterna fossæ cerebri lateralis is formed in front of either temporal lobe by the arachnoid bridging across the lateral fissure. This cavity contains the middle cerebral artery. • The cisterna venæ magnæ cerebri occupies the interval between the splenium of the corpus callosum and the superior surface of the cerebellum; it extends between the layers of the tela chorioidea of the third ventricle and contains the great cerebral vein. • A series of six selected axial Tl C+ MR images through brain from inferior to superior shows normal meningeal enhancement at l.ST. Unlike arachnoid microvessels, dural microvessels lack capillary endothelial tight junctions. Dural enhancement is therefore normal following contrast administration. • (Middle) The outer and inner dural layers adhere to each other, except where encase dural venous sinuses. Venous flow in sinuses is relatively slow so strong enhancement is normal. A small arachnoid granulation is present, seen here as a CSF-intensity filling defect within the strongly enhancing sinus confluence. • (Bottom) The falx cerebri encases the superior and inferior sagittal sinuses at its upper and lower margins respectively. The V-shaped tentorial apex is seen very well on this image. Note inhomogeneous signal within the superior sagittal sinus, a normal finding. 23/02/2016 14 • (Top) Normal dural enhancement is thin, smooth, discontinuous and symmetric (best appreciated on coronal sections). Enhancing superficial cortical veins travel within subarachnoid space before traversing potential subdural space to drain into dural sinuses. Superficial cortical veins are typically seen as thicker, more strongly enhancing structures that branch and communicate with draining tributaries extending into sulci. • (Middle) Section through the centrum semiovale shows the falx cerebri with a prominent inferior sagittal sinus arcing above the corpus callosum. • (Bottom) Scan through the vertex shows the triangular- shaped superIor sagittal sinus, which is larger posteriorly than anteriorly.
