What are the steps to identify and approach shock?
Step 1: Assess circulation, pulses, skin color, temperature, moisture, capillary refill, and livedo reticular.
Step 2: Check peripheral and central pulses (umbilical, brachial, femoral, carotid) and evaluate their strength (present, strong, weak, fine, absent).
Step 3: Check heart rate, blood pressure, and volume status (oral intake, hydration, diuresis and urinary output, external bleeding).
Step 4: Identify shock by assessing low perfusion and cardiovascular system failure.
Step 5: Classify the etiology of shock as hypovolemic, cardiogenic, distributive (septic, anaphylactic, neurogenic), or obstructive.
Step 6: Approach shock by ensuring airway, breathing, and circulation; monitoring; venous access; and volume resuscitation (10-20 mL/kg of crystalloid in 20 minutes, repeated only if it does not generate volume overload). Ensure hydration, normothermia, and normoglycemia. Initiate vasopressor therapy and adjust the dose as necessary. Treat the underlying cause.
