Lidocaine 2% with epinephrine 1:100,000 or Prilocaine with felipressin 0.03 UI/ml with a maximum limit of 2 cartridges is recommended for hypertensive patients. The use of vasoconstrictors is accepted in controlled patients, but contraindicated in patients with recent myocardial infarction, severe heart failure, uncontrolled hypothyroidism, or severe uncontrolled hypertension. In emergency situations with decompensated patients, Mepivacaine 3% without vasoconstrictor is recommended.
Prilocaine with felipressin 1:100,000 or Lidocaine with norepinephrine 1:100,000 with a maximum limit of 2 cartridges is recommended for diabetic patients. Epinephrine is contraindicated due to the breakdown of glycogen into glucose, which can result in hyperglycemia.
